2009-2014年淮安市流动人口结核病防治现状分析

摘　　要：目的了解淮安市流动人口肺结核发病与治疗管理情况,为开展流动人口结核病防治工作提供理论依据。方法对2009-2014年淮安市登记管理的402例流动人口肺结核患者基本情况、临床特征、治疗转归情况进行分析。结果2009-2014年淮安市流动人口肺结核患者中男性多于女性,以青壮年为主,占73.63%;职业分布以民工居多,占75.12%,学生结核病患者占10.20%。转诊/追踪是患者主要来源方式,占41.54%,其次为健康检查,占32.09%,因症就诊比例较小,占20.65%;63.93%的患者出现就诊延迟。72.14%的患者初诊伴咳嗽、咳痰症状,31.09%的患者痰结核分枝杆菌阳性,29.60%的患者胸片显示肺部有空洞。患者成功治疗率为89.30%,系统管理率为93.78%。结论淮安市流动人口结核病患者以青壮年男性为主,患者主动就诊意识及治疗依从性较差,因此应加强对流动人口青壮年劳动力及学生的结核病防控工作。Objective To know the situations of incidence and treat-management of tuberculosis（TB） in floating population in Huaian City from 2009 to 2014, so as to provide theoretical evidence for TB control and prevention in floating population. Methods We analyzed the base situations, clinical characteristics and outcomes of treatment and transformation among 402 cases of floating population with TB registered in Huaian City from 2009 to 2014. Results Among the TB patients of floating population from 2009 to2014, males were more than females, and the cases were mainly young people which accounted for 73.63%. The distribution of occupation of TB patients was mainly migrant workers which accounted for 75.12%, while the students accounted for 10.20%. The main source of patients was transferring or tracing which accounted for 41.54%, following ed health examination which accounted for32.09%, while the proportion of patients seeing the doctor initiatively accounted for 20.65%. 63.93% patients were late to see the doctors. 72.14% patients were diagnosed with the symptom of cough. 31.09% patients were smear positive, and 29.60% cases were diagnosed with holes in the lung. The rate of successful treatment was 89.30%, and the rate of systematical management was93.78%. Conclusion Most of TB patients in the floating population were the young men in Huaian City who lack of consciousness of active treatment and compliance, so the work of TB prevention and control on the young migrant labors and students should be strengthened.
