超声在评价老年性心力衰竭患者左心形态与功能中的作用

摘　　要：目的探讨超声在评价老年性心力衰竭患者左心形态与功能中的作用。方法选择心力衰竭的老年患者共90例,分为收缩性心力衰竭组(51例),舒张性心力衰竭组(39例),并匹配对照组50例。采用心彩超检测左心形态与功能指标并比较。结果舒张性心力衰竭的患者左心房内径、室间隔厚度(IVST)、左心室后壁厚度显著高于对照组,二尖瓣舒张早期流速峰值/二尖瓣舒张晚期流速峰值(E/A)显著低于对照组(P<0.05)。舒张性心力衰竭的患者IVST、左心室后壁厚度,左心室射血分数、心脏指数显著高于收缩性心力衰竭的患者,左心房内径显著低于收缩性心力衰竭的患者(P<0.05)。收缩性心力衰竭的患者左心房内径、左心室内径、左心室后壁厚度显著高于对照组,左心室射血分数、心脏指数、E/A显著低于对照组(P<0.05)。结论超声在评价老年舒张性心力衰竭与收缩性心力衰竭的心脏形态和功能中具有较好的作用,值得临床推广和运用。Objective To explore the role of ultrasound in evaluation of left ventricular function in elderly patients with heart failure.Methods A total of 90 elderly patients with heart failure were collected and divided into systolic heart failure group（ 51 cases）,diastolic heart failure group（ 39 cases）,and matched control group（ 50 patients）. Left ventricular morphology and function parameters were detected by using heart ultrasound and compared. Results The diastolic heart failure,left atrial diameter,IVST and left ventricular wall thickness in patients with diastolic heart failure were significantly higher than those of patients in control group,and E / A was significantly lower than that of control group.IVST,thickness of left ventricular wall,left ventricular ejection fraction and cardiac index in patients with diastolic heart failure were significantly higher than those of patients with systolic heart failure,and left atrial diameter was significantly lower in patients with systolic heart failure. Left atrial diameter,left ventricular diameter and left ventricular wall thickness in patients with systolic heart failure were significantly higher and left ventricular ejection fraction,cardiac index and E / A were significantly lower than those of control group. Conclusion Ultrasound has advantages in evaluation of diastolic heart failure,systolic cardiac morphology and function in elderly patients with heart failure,hence it is worthy to be promoted and applied.
