益气活血方对非ST段抬高型心肌梗死合并心源性休克患者血流动力学参数的影响  

Effects of Yiqi Huoxue Prescription on Hemodynamics in Patients with Non-STsegment Elevation Myocardial Infarction and Cardiac Shock

摘　　要：目的研究益气活血方对非ST段抬高型心肌梗死合并心源性休克的血流动力学参数的影响。方法将24例患者随机分为治疗组和对照组各12例,均以西医常规对症治疗,治疗组加用益气活血中药汤剂胃管鼻饲治疗1周,比较两组患者治疗前、治疗3d后及治疗1周后的心指数(cardiac index,CI)、中心静脉压(central venous pressure,CVP)、血管外肺水指数(extravascular lung water index,ELWI)、全心舒张末容积指数(end diastolic volume index,GEDI)、外周血管阻力指数(systemic vascular resistance index,SVRI)和平均动脉压(mean arterial pressure,MAP)的变化。结果治疗1周后,两组CI、SVRI、GEDI较治疗前显著升高(P<0.05),ELWI较治疗前显著降低(P<0.05);治疗1周后,治疗组CI、SVRI显著高于对照组(P<0.05),ELWI显著低于对照组(P<0.05);两组同时点CVP、MAP、GEDI比较,差异均无统计学意义(P>0.05)。结论在西医常规治疗基础上加用益气活血中药可改善非ST段抬高型心肌梗死合并心源性休克的血流动力学状况,其效应随治疗时间延长而增加。Objective To study the effects of Yiqi Huoxue Prescription on hemodynamics in patients with non-ST-segment elevation myocardial infarction and cardiac shock. Methods Twenty-four patients were randomly and equally divided into treatment group and control group. Both groups were treated with conventional Western medicine therapy. The treatment group was additionally treated with Yiqi Huoxue Decoction through a nasal feeding tube for one week. The cardiac index （CI）, central venous pressure （CVP）, extravascular lung water index（ELWI）, global end-diastolic volume index （GEDI）, systemic vas cular resistance index （SVRI）, and mean arterial pressure （MAP） in the two groups were evaluated before and after three days and one week of treatment. Results After one week of treatment, CI, SVRI, and GEDI were significantly elevated in the two groups （P〈0.05）, while ELWI was significantly reduced in both groups （P〈0.05） ; the treatment group had significantly elevated SVRI and CI and significantly reduced ELWI compared with the control group （P〈0.05） ; there were no significant differences in CVP, MAP, and GEDI between the two groups （P〈0.05）. Conclusion Yiqi Huoxue Prescription in addition to conventional Western medicine therapy can improve the hemodynamic status in patients with non-ST-segment elevation myocardial infarction and cardiac shock. Patients with a longer treatment duration achieve a better outcome.
