原发性高血压患者血压变异性与左心室舒张功能的相关性研究

摘　　要：目的探讨原发性高血压患者血压变异性与左心室舒张功能的相关性。方法 145例原发性高血压患者,依据彩色多普勒超声心动图检测结果分为舒张功能正常组(88例),不全组(57例)。应用动态血压监测仪连续24 h检测血压,并计算血压变异性(BPV)各指标。比较2组超声心动图各检测指标及动态血压参数及BPV各指标。对BPV各指标与左心室舒张功能不全进行多因素Logistic回归分析。结果舒张功能正常组E、A、E/Ea低于不全组,Ea、A'高于不全组(P<0.05)。正常组24 h平均舒张压、白天及夜间平均收缩压、夜间平均舒张压、SBPSD、DBPSD、DBPCV均明显低于不全组(P<0.05)。SBPSD、SBPCV与左心室舒张功能不全呈正相关(r=0.119,P<0.01)。结论血压变异性是左心室舒张功能的独立影响因素之一,降低血压变异性对改善左心室舒张功能具有重要意义。Objective To explore the correlation between blood pressure variability（ BPV）and left ventricular diastolic function in patients with primary hypertension. Methods A total of 145 patients with primary hypertension were divided into flow-mediated vasodilation（ FMV） normal group（ 88 cases） and FMV dysfunction group（ 57 cases）,according to the detection results of color doppler echocardiography. Ambulatory blood pressure recorder was applied to detect the continuous 24-h blood pressure,and BPV indexes were calculated. The echocardiography indexes,ambulatory blood pressure indexes and BPV indexes were compared between two groups. And then multivariate Logistic regression analysis was conducted to analyze the relationship between BPV indexes and left ventricular diastolic dysfunction. Results FMV normal group was evidently lower in E,A and E / Ea but markedly higher in Ea and A＇than FMV dysfunction group（ P〈0.05）. And FMV normal group was prominently lower in 24-h diastolic blood pressure（ DBP）,daytime and night mean systolic blood pressure（ SBP）,night mean DBP,SBP standard deviation（ SBPSD）,DBPSD and DBP coefficient of variability（ DBPCV） than FMV dysfunction group（ P〈0.05）. Additionally,SBPSD and SBPCV were in positive association with left ventricular diastolic dysfunction（ r = 0. 119,P〈0.01）. Conclusion BPV is one of the independent influencing factors for left ventricular diastolic function,so reducing BPV is of great significance in improving left ventricular diastolic function.
