气道分级管理在ICU机械通气患者中的应用

摘　　要：目的探讨气道分级管理在ICU机械通气患者中的临床应用效果。方法收集85例ICU机械通气患者按是否愿意接受气道分级管理分为2组,对照组患者给予常规胸部物理治疗,实验组患者则按照气道分级管理模式进行干预,比较2组患者机械通气相关指标、相关临床指标、机械通气相关并发症与护理满意度等。结果实验组患者潮气量与氧合指数显著性高于对照组,呼吸频率与APACHEⅡ评分显著性低于对照组,差异有统计学意义(P<0.01);实验组患者通气时间与住院时间显著性低于对照组,差异有统计学意义(P<0.01);实验组患者谵妄、VAP与医疗纠纷发生率明显低于对照组,差异有统计学意义(P<0.05);实验组患者护理满意度明显高于对照组,差异有统计学意义(P<0.05)。结论气道分级管理在ICU机械通气患者中的临床应用效果显著,可提高满意度。Objective To evaluate the clinical effect of airway hierarchical management in ICU patients with mechanical ventilation. Methods A total of 85 hospitalized ICU patients with mechanical ventilation were divided into two groups according to their willing to accept the airway hierarchical management. The control group was treated with conventional chest physical therapy,and the experimental group was given airway management interventions. Related indicators of mechanical ventilation,mechanical ventilation related complications and nursing satisfaction were compared. Results The tidal volume and oxygenation index in the experimental group was significantly higher,respiratory frequency and APACHE Ⅱ score were significantly lower than that in the control group,there were significant difference（ P〈0. 01）. Ventilation time and length of hospital stay in the experimental group was significantly lower than that in the control group,（ P〈0. 01）. The delirium,VAP and incidence of medical disputes in the experimental group were significantly lower than that in the control group,there was a significant difference（ P〈0. 05）. The satisfaction in the experimental group was significantly higher than that in the control group,there was a significant difference（ P〈0. 05）.Conclusion Airway hierarchical management can significantly improve satisfaction of mechanical ventilation in ICU patients.
