缬沙坦、福辛普利钠及苯磺酸氨氯地平治疗高血压合并轻度认知功能障碍患者的对比研究

摘　　要：目的探讨缬沙坦、福辛普利钠及苯磺酸氨氯地平治疗高血压合并轻度认知功能障碍(MCI)的临床疗效。方法将90例高血压合并MCI患者随机分为3组,每组30例。3组均服用阿司匹林肠溶片。在此基础上,缬沙坦组给予缬沙坦胶囊,福辛普利钠组给予福辛普利钠片,苯磺酸氨氯地平组给予苯磺酸氨氯地平片,1次/d。血压达标后,均连续治疗12个月。观察3组治疗前及治疗后12个月简易精神状态量表(MMSE)及蒙特利尔认知评估量表(Mo CA)评分的变化,并对治疗后12个月临床疗效进行评定。结果治疗12个月后,3组MMSE及Mo CA评分均较治疗前明显升高(P<0.05或P<0.01),而3组间比较差异无统计学意义(P>0.05)。缬沙坦组、福辛普利钠组和苯磺酸氨氯地平组MMSE、Mo CA总有效率分别为30.0%和36.7%、20.0%和20.0%、16.7%和16.7%,缬沙坦组略高于其余2组,但3组间比较差异无统计学意义(P>0.05)。结论缬沙坦、福辛普利钠和苯磺酸氨氯地平3种药物在降压达标后均可改善高血压患者伴有的MCI,其中缬沙坦改善作用较为明显。Objective To explore the clinical efficacy of valsartan,fosinopril and amlodipine besylate in treatment of hypertension patients complicated with mild cognitive impairment（ MCI）.Methods Ninety hypertension patients complicated with MCI were randomly divided into 3 groups,30 cases in each group. The patients in all groups were treated with aspirin enteric-coated tablet.Based on this,valsartan group,fosinopril group and amlodipine besylate group were respectively given valsartan capsules,fosinopril sodium tablets and amlodipine besylate tablets,once a day. After the blood pressure reached the standard,all the patients were treated continuously for 12 months. The changes of mini-mental state examination（ MMSE） and montreal cognitive assessment（ Mo CA）scores were observed in 3 groups before treatment and 12 months after treatment,and the clinical efficacy was also evaluated 12 months after treatment. Results Twelve months after treatment,both MMSE and Mo CA scores in 3 groups went up dramatically when compared with those before treatment（ P〈0. 05 or P〈0. 01）,but there were no significant differences among 3 groups（ P〈0. 05）. The overall response rates of MMSE and Mo CA in valsartan group,fosinopril group and amlodipine besylate group were 30. 0% and 36. 7%,20. 0% and 20. 0% as well as 16. 7% and 16. 7% respectively,and those indexes in valsartan group were slightly higher than other two groups（ P〈0. 05）.Conclusion When the blood pressure is controlled in the standard range,valsartan,fosinopril and amlodipine besylate can ameliorate MCI in patients with hypertension,and the improvement effect of valsartan is the most significant.
