预防性放疗在食管癌根治术预后中的价值探讨

摘　　要：目的分析食管癌根治术后预防性放射治疗的临床价值。方法收集诊治的食管癌患者70例作为研究对象,按照患者意愿分为观察组和对照组,每组35例,对照组患者单纯给予手术治疗,观察组患者手术后给予预防性放射治疗,对两组患者的治疗效果进行对比分析。结果截止到2015年4月1日,全组中位生存时间25.3个月。观察组患者1、3年生存率分别为88.57%(31/35)、80.00%(28/35),对照组患者1、3生存率分别为77.14%(27/35)、57.14%(20/35);两组患者根治术后1、3年生存率比较,差异有统计学意义(P<0.05);术后观察组淋巴结转移率、吻合口复发率明显小于对照组,组间比较差异有统计学意义(P<0.05);两组血行转移率比较无统计学差异(P>0.05)。结论对根治手术的食管癌患者术后行预防性放疗可有效降低患者术后局部复发的发生率,有利于食管癌患者预后。Objective To analyze the clinical value of prophylactic radiotherapy in the patients after radical surgery for esophageal carcinoma. Methods Seventy patients with esophageal carcinoma selected from our hospital were divided into an observation group and a control group according to their willing,with 35 cases in each group. Control group received surgical treatment alone,while observation group was given surgical treatment and prophylactic radiotherapy. The therapeutic effect of patients in the two groups was compared. Results Until April 1st,2015,the median survival time of all patients was 25. 3 months. The 1- year and 3- year survival rates were 88. 57%（ 31 /35） and 80. 00%（ 28 /35）in the observation group,while the percentages were 77. 14%（ 27 /35） and 57. 14%（ 20 /35） in the control group.There were statistically significant differences in the 1- year and 3- year survival rate after radical surgery between the two groups（ P 〈0. 05）. The incidence of lymph node metastasis and anastomotic recurrence rate in the observation group were significantly lower than the control group（ P 〈0. 05）. No significant difference was found in the hematogenous metastasis rate between the two groups（ P 〉0. 05）.Conclusions Prophylactic radiotherapy can effectively reduce the incidence of local recurrence and improve the prognosis of the patients with esophageal carcinoma.
