HPV感染状态与不同级别宫颈病变感染状态的关系

摘　　要：目的探讨人乳头瘤病毒(HPV)在不同级别宫颈病变中的感染状态,HPV和TCT检查在宫颈病变筛查中的意义。方法收集2013年6月至2014年1月就诊于我院妇科门诊564名患者的资料,应用回顾性研究方法 ,分析HPV总体感染情况,比较不同基因型HPV分布及感染率、不同病理级别宫颈病变患者HPV感染情况及基因型分布。结果 564例患者中有409例HPV感染者,HPV总感染率为72.5%,其中单一型别感染率为35.82%,混合型别感染率为23.7%,感染率排名前五位的亚型是:HPV 16(26.3%)、HPV 18(15.3%)、HPV 52(9.6%)、HPV 53(8.2%)、HPV 58(4.7%)。正常宫颈、CIN-Ⅰ级、CINⅡ级、宫颈癌患者的HPV感染率分别为75.8%、90.1%、95.0%,显著高于正常组66.6%(χ2=4.101,24.93,6.829,P<0.05)。结论各宫颈病变组间的HPV感染率有统计学差异。随着宫颈病变级别的增加,HPV的感染率也增加。Objective To investigate human papillomavirus HPV infection status indifferent grade cervical lesion the value of cervical liquid based cytology TCT and human papillomavirus HPV testing for the screening of cervical lesions. Methods From June 2013 to January 2014,clinical data of 564 patients received by gynecology clinic of our Hospital were collected. A retrospective analysis was carried out to analyze overall prevalence of HPV infection. And the prevalence of HPV infection in different subtypes was compared. And the prevalence of HPV infection and the distribution of HPV types in different pathological grade of cervical lesions were analyzed. Results Among the 564 patients,409 cases were infected with HPV, the overall HPV prevalence was 72. 5%. The single subtype infection rate was 35. 82%,the multiple subtypes infection rate was23. 7 %. The most common genotypes were HPV 16（ 26. 3 %）,HPV 18（ 15. 3%）,HPV 52（ 9. 6%）,HPV53（ 8. 2%） and HPV 58（ 4. 7 %）. The prevalence of HPV infection in CIN-Ⅰ,CIN-Ⅱ,and cervical cancer group were 75. 8%,90. 1%,and 95. 0%,was higher than the normal 66. 6%（ χ-2= 4. 101,24. 93,6. 829,P〈 0. 05）,respectively. There was significant difference in HPV prevalence between normal cases and cervical abnormalities（ P〈 0. 05）. Conclusions There is significant difference in HPV prevalence between five cervical lesion groups（ P〈 0. 05）. With the increase of cervical lesion level,HPV infection rate also increases.
