健脾调肝温肾方对腹泻型肠易激综合征患者肠黏膜5-羟色胺及其受体的影响

摘　　要：目的:观察腹泻型肠易激综合征(D-IBS)患者肠黏膜中5-HT浓度及5-HT3受体mRNA表达的变化,探讨健脾调肝温肾方的作用机制。方法:选择80例D-IBS患者,随机分为中药治疗组与西药对照组各40例,选取健康体检肠镜检查者10例作为正常对照组。中药治疗组口服健脾调肝温肾方并随证加减,西药对照组口服匹维溴铵片;疗效为4周。应用双抗体夹心法测定肠黏膜5-HT浓度,应用实时定量聚合酶链反应(RT-PCR)检测5-HT3受体mRNA的表达水平。结果:两组治疗前肠黏膜5-HT浓度增高,5-HT3受体表达增强,应用健脾调肝温肾方及匹维溴铵治疗可以明显降低肠黏膜5-HT浓度,减少5-HT3受体表达;两组治疗后肠黏膜5-HT浓度及5-HT3受体表达与正常对照组比较无明显差别。结论:健脾调肝温肾方通过减少肠黏膜5-HT释放,减少5-HT3受体表达,调控神经介质和免疫细胞关系而改善IBS内脏高敏性,减轻或缓解临床症状。Objective： To observe the changes of 5- HT concentration and 5- HT3 receptor mRNA expression in intestinal mucosa of patients with Diarrhea- predominant irritable bowel syndrome,and investigate the mechanism of Jianpi Tiaogan Wenshen Fang（ JTWF）. Methods： With a prospective,randomized controlled trial adopted,80 patients with D- IBS were assigned randomly equally to two groups,in addition,10 healthy volunteers were chosen as the normal group. The treatment group was treated with JTWF,and the control group was treated with pinaverium bromide tablet（ PVB）,all for four weeks. Using double antibody sandwich method to determine the concentration of 5- HT in intestinal mucosa,and using real- time quantitative polymerase chain reaction to detect mRNA 5- HT3 receptors expression level. Results： The concentration of 5- HT increased and the expression of 5- HT3 receptor enhanced in the intestinal mucosa of the traditional Chinese medicine treatment group and Western medicine control group before treatment to compare with normal people; JTWF and pinaverium bromide could obviously reduce the intestinal 5- HT concentration and reduce the expression of 5- HT3 receptor,the expressions of two groups were simllar to that in the normal control group,there was no significant difference. Conclusion： Jianpi Tiaogan Wenshen Fang can alleviate clinical symptoms by reducing intestinal mucosal 5- HT concentration and the expression of 5- HT3 receptor,regulate the relationship of neurotransmitter and immune cells to improve IBS splanchnic hypersensitivity.
