2型糖尿病患者抗心磷脂抗体水平与并发症发生率的临床研究

摘　　要：目的观察2型糖尿病患者血清中抗心磷脂抗体（ACL）水平与糖尿病相关并发症发生率的相关性。方法收集2014年6月-2015年6月就诊于我院的2型糖尿病患者180例（平均年龄44±17岁）,其中合并血管并发症患者为A组（112例）,未合并血管并发症患者为B组（68例）,健康对照组为C组（180例,平均年龄41±19岁）,记录所有入组个体的基本资料,检测空腹血糖,血脂,糖化血红蛋白,24 h尿蛋白,尿蛋白/肌酐,肾小球率过滤（GFR）。采用酶联免疫吸附法测定所有入组个体血清中抗心磷脂抗体（ACL,Ig G及Ig M亚型）的水平。结果 A、B两组患者血清中Ig G ACL及Ig M ACL抗体的水平高于C组（P〈0.001）,且A组患者Ig M ACL水平高于B组及C组（P〈0.05）,但A、B组间Ig G ACL水平无差异。结论抗心磷脂抗体水平在合并血管并发症的糖尿病患者中增高,表明ACL可能参与了2型糖尿病的血管并发症的发生。Objective To observe the prevalence of anticardiolipin antibodies（ACL）antibodies in type 2 diabetes patients with and without vascular complications. Methods 180 adults with type 2 diabetes mellitus patients were enrolled in this study,including 118 subjects with vascular complications（group A）and 68 subjects without vascular complications（group B）,the control group（group C）was composed of 180 age and sex-matched healthy individuals. The basic information of each subject was recorded. Fasting blood glucose,blood lipid levels,Hb A1 c,Estimated GFR,albuminuria,albumin-to-creatinine ratio and ophthalmologists of each subject were measured. The levels of ACL immunoglobulin Ig G and ACL Ig M antibodies were measured by enzyme linked immunosorbent assays. Results Compared with group C,anti-cardiolipin antibodies of Ig G and Ig M were significantly increased in in group A and B（P〈0.001）,and Ig M a CL in group A was higher than group B and C（P〈0.05）,but the level of Ig G ACL was no difference between patients with and without vascular complications（P〈0.05）. Conclusion The higher prevalence and levels of anticardiolipin antibodies in patients with vascular complications suggests that these humoral factors might be involved in the vascular complications of type 2 diabetes mellitus.
