垂体后叶素水分离法在腹腔镜卵巢子宫内膜异位囊肿剥除术中减轻卵巢损伤的疗效观察

摘　　要：目的探讨垂体后叶素水分离法在腹腔镜卵巢子宫内膜异位囊肿剥除术中对卵巢功能影响。方法选取2014年2月至2015年2月我院收治78例随机分为研究组和对照组,研究组术中局部使用稀释垂体后叶素,对照组不使用垂体后叶素,比较两组患者手术时间、术中出血量、肛门排气时间,对两组患者随访3月,比较两组患者卵泡刺激素(FSH)、黄体生成素(LH)、雌二醇(E2)、卵巢储备功能降低和复发率。结果研究组患者手术时间、术中出血量均少于对照组[(49.5±16.2)min比(78.6±17.0)min,(65.8±16.7)m L比(109.7±17.9)m L,P<0.05]。研究组患者术后3月FSH、LH水平均低于对照组,E2水平高于对照组[(6.5±2.1)m IU/m L比(7.4±1.8)m IU/m L,(5.4±1.2)m IU/m L比(6.3±1.1)m IU/m L,(78.4±2.6)pmol/L比(58.4±3.1)pmol/L,P<0.05]。研究组患者术后3月卵巢储备功能降低及复发发生率均低于对照组[5.13%比28.21%,2.56%比23.08%,P<0.05]。结论垂体后叶素水分离法应用于腹腔镜卵巢子宫内膜异位症囊肿剥离术中可缩短患者手术时间、降低术中出血量,可降低对卵巢功能影响,降低术后复发率。Objective To investigate the impact of vasopressin after water separation in laparoscopic ovarian cystectomy endometriosis in ovarian function. Method Select 78 cases in February 2014-February 2015 in our hospital,randomly divide them to study group and control group,intraoperative topical study group after dilution vasopressin,control group did not use vasopressin,compare the two groups of patients in operative time,blood loss,anal exhaust time,two groups of patients were followed up in March,compare the two groups of patients in follicle stimulating hormone（ FSH）,luteinizing hormone（ LH）,estradiol（ E2）,ovarian reserve reduction and relapse rate. Result Operation time,blood loss,the study group were less than the control group [（ 49. 5 ± 16. 2） min vs.（ 78. 6 ± 17. 0） min,（ 65. 8 ± 16. 7） ml vs.（ 109. 7 ± 17. 9） ml,P〈0. 05]. March postoperative FSH,LH level study group were lower than the control group,E2 levels higher than [（ 6. 5 ± 2. 1） m IU / m L vs.（ 7. 4 ± 1. 8） m IU / m L,（ 5. 4 ± 1. 2） m IU/ m L vs.（ 6. 3 ± 1. 1） m IU / m L,（ 78. 4 ± 2. 6） pmol / L vs.（ 58. 4 ± 3. 1） pmol / L,P〈0. 05]. March postoperative ovarian reserve and reduce the incidence of relapse study group were lower than the control group[5. 13% vs. 28. 21%,2. 56% vs. 23. 08%,P〈0. 05]. Conclusion Vasopressin water separation method is applied laparoscopic ovarian endometriosis cyst dissection can be shortened operation time,reduce blood loss,can reduce the impact on ovarian function,reduced the recurrence rate.
