非诺贝特治疗脂蛋白肾病尿检完全缓解3例报告及文献复习

摘　　要：目的 观察非诺贝特治疗脂蛋白肾病（LPG）的疗效.方法 回顾性观察南京军区总医院经肾活检确诊的LPG患者40例,其中3例经非诺贝特治疗后尿检完全缓解,分析这3例患者的临床病理表现及治疗反应.结果 3例患者肾脏表现均为中等量蛋白尿,肾功能正常,血白蛋白正常或偏低,实验室检查有血浆载脂蛋白（Apo）E升高,肾活检可见肾小球内大量脂蛋白栓子堵塞毛细血管及ApoE染色阳性,3例患者经非诺贝特等药物治疗后,短期内尿蛋白显著减少,随访1年蛋白尿完全缓解,肾功能长期稳定.结论 非诺贝特可以降低脂蛋白肾病患者的蛋白尿,部分患者尿检可完全缓解.Objectives To observe the curative effect of fenofibrate on lipoprotein nephropathy （LPG）.Methods 40 patients with LPG confirmed by renal biopsy were retrospectively studied.In three cases,their proteinuria turned negative after treated with fenofibrate.The clinical manifestations and treatment responses were analysised.Results Three patients presented with moderate proteinuria、 ormal renal function and elevated plasma apolipoprotein E （Apo）, renal biopsy disclosed many lipoprotein embolus in the glomerular capillary, and the ApoE staining were positive.After treatment with fenofibrate, urinary protein significantly reduced.After 1 year of follow-up, their proteinuria got complete remission and the renal function was stable.Conclusions Our clinical observation showed that fenofibrate could improve the level of proteinuria in patients with lipoprotein nephropathy, in some patients, the proteinuria could get completely remission.
