外剥内扎结合皮下痔静脉丛剥离术治疗环形混合痔的临床效果观察

摘　　要：目的观察外剥内扎结合皮下痔静脉丛剥离术治疗环形混合痔的临床效果。方法选取2013年5月~2014年12月在辽宁中医药大学附属医院肛肠科治疗的患者90例,随机分为实验组及对照组,每组45例。实验组给予外剥内扎结合皮下痔静脉丛剥离术治疗,对照组给予分段结扎术治疗,观察临床疗效及术后近期、远期并发症情况。结果实验组的近期并发症（便血、水肿、疼痛）、远期并发症（肛门狭窄、便血、复发）发生率明显低于对照组,创面愈合时间明显短于对照组,差异有统计学意义（P〈0.05）。结论外剥内扎结合皮下痔静脉丛剥离术治疗环形混合痔的临床效果优于分段结扎术,可减少术后并发症,缩短愈合时间,提高治愈率,是一种值得推广及应用的术式。Objective To observe the clinical effect of external dissection and internal ligation combined with subcutaneous hemorrhoid venous plexus decollement in the treatment of annular mixed hemorrhoids. Methods From May 2013 to December 2014,90 patients treated by department of anorectum in our hospital were selected and they were randomly and evenly divided into experimental group and control group.Experimental group was given external dissection and internal ligation combined with subcutaneous hemorrhoid venous plexus decollement while control group was given segmented ligation.The clinical curative effect,short and long-term complication after surgery was observed respectively.Results In the experimental group,the occurrence rate of short-term complication（hematochezia,edema,and pain） and short-term complication（stricture of anus,hematochezia and recurrence） was obviously lower than that of control group respectively,and wound surface healing time was obviously shorter than that of control group,and there was a statistical difference（P〈0.05）. Conclusion Clinical effect of external dissection and internal ligation combined with subcutaneous hemorrhoid venous plexus decollement in the treatment of annular mixed hemorrhoids is better than that by segmented ligation,and it can reduce postoperative complications,shorten healing time,improve the cure rate,and this surgical method is worthy of promotion and application.
