摘 要:目的探讨Graves病甲亢患者血清甲状腺激素水平与血脂水平之间的相关性。方法回顾分析50例Graves病甲亢患者(GD组)与25例健康对照者(对照组)的游离三碘甲腺原氨酸(FT3)、游离甲状腺素(FT4)、促甲状腺激素(TSH)、甘油三酯(TG)、总胆固醇(TC)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)、载脂蛋白AⅠ(Apo AⅠ)、载脂蛋白B(Apo B)检测结果,FT3、FT4、TSH测定采用电化学发光法,TG、TC测定采用酶法,HDL-C、LDL-C、Apo AⅠ、Apo B测定采用免疫透射比浊法。结果 (1)GD组的血清FT3、FT4、Apo AⅠ显著高于对照组,TSH、TC、HDL-C、LDL-C、Apo B显著低于对照组,两组比较差异有极其显著的统计学意义(P<0.01)。两组患者的TG相比差异无统计学意义(P>0.05)。(2)Graves病甲亢患者的血清TC、LDL-C、Apo B与FT3、FT4之间呈负相关,与TSH之间呈正相关;而血清TG、Apo AⅠ、HDL-C与FT3、FT4、TSH之间无相关。结论 Graves病患者存在明显的血脂代谢紊乱,TC、LDL-C、Apo B等项目的紊乱程度与患者甲状腺激素水平呈一定的关系,因此这些项目可以作为判断Graves病患者病情的指标。Objective To investigate the relationship between serum thyroid hormone levels and blood lipids levels in patients with Graves' disease. Methods The serum free triiodothyronine (FT3), free thyroxine(FT4 ), thyroid stimu- lating hormone (TSH), triglyceride(TG), total cholesterol(TC), high-density lipoprotein cholesterol(HDL-C),low-den- sity lipoprotein cholesterol(LDL-C), apolipoprotein A I (Apo A I ), apolipoprotein B(Apo B) results of 50 patients with Graves' disease (GD group) and 25 normal subjects (control group) were retrospectively analyzed. FT3, FT4 and TSH were measured by electrochemilumineseence immunoassay, TG and TC by enzymatic method, HDL-C, LDL-C, Apo A I and Apo B by immune turbidimetric method. Results GD group had lower concentrations of FT3, FT4 and Apo A I , but higher TSH, TC, HDL-C, LDL-C and Apo B concentrations compared with control group (P〈0.01). There was no significant difference in TG between GD group and control group (P〉0.05). Serum TC, LDL-C and Apo B were negatively correlated with FT3 and FT4, and positively correlated with TSH in patients with Graves' disease, but serum TG, Apo:A I and HDL-C were not correlated with FT3, FT4 and TSH. Conclusion Patients with Graves' disease have lipid metabolism disorders. There are certain relationships between TC, LDL-C and Apo B levels and serum thyroid hor- mone levels, so these indexes are able to be used as indicators for patients with Graves' disease.
