摘 要:目的:观察芪黄煎剂对大鼠胃切除后肠淋巴细胞归巢受体α_4β_7、L-selectin及LFA-1的影响。方法:将60只大鼠随机分成假手术组、对照组和芪黄煎剂组,每组20只,假手术组仅予以大鼠腹部皮肤切开后缝合,不行胃切除,不给予肠内营养和芪黄煎剂;对照组大鼠行胃切除后给予肠内营养制剂(能全素);芪黄煎剂组大鼠胃切除后滴注肠内营养制剂和芪黄煎剂,连续给药1周后,分离肠淋巴管主干,并收集肠淋巴液,采用流式细胞术检测归巢受体α_4β_7、L-selectin和LFA-1的阳性表达数,并计算出单个核细胞的阳性表达率;RT-PCR荧光定量法检测引流肠淋巴液中归巢受体的mRNA相对表达量。结果:与假手术组比较,对照组和芪黄煎剂组淋巴受体细胞的阳性表达均显著下降(P<0.01);与对照组比较,芪黄煎剂组的阳性细胞数量较多,但差异无统计学意义(P>0.05);与假手术组比较,芪黄煎剂组和对照组肠淋巴液中归巢受体的mRNA相对表达量均显著降低(P<0.01);与对照组比较,芪黄煎剂组的mRNA相对表达量显著增多(P<0.05);实验发现受体LFA-1表达数量较α_4β_7、L-selectin增多显著(P<0.01)。结论:芪黄煎剂通过增加肠淋巴细胞表面归巢受体α4β7、L-selectin和LFA-1阳性细胞数量及mRNA相对表达量,使肠道固有层和上皮内黏膜地址素更好地捕获到受体,改善胃切除创伤后肠黏膜免疫屏障功能。Objective: To study the effects of Qihuang Decoction(芪黄煎剂) on lymphocyte homing receptors of α_4β_7, L-selectin and LFA-1 after gastric operation in rats. Methods: The 60 rats were randomly divided into sham operation group, control group and Qihuang Decoction group. The rats in the sham operation group were only treated with an operation of abdominal middle line incision and suture with the stomach retaining.The rats in the control group were treated with an operation of stomach excision and Nutrison(an enteral nutrition preparation) after operation, an intacted protein enteral nutrition powder. The rats in the Qihuang Decoction group were given Nutrison and Qihuang Decoction after an operation of stomach excision. The treatment course was 1 week. At the end of the course, the intestinal lymph conduit was isolated,gathered the lymph fluid in the lymph conduit. The express-quantity of homing receptors of α_4β_7, L-selectin and LFA-1 were detected, and the positive rate of mononuclear cells in lymph fluid was counted by using flow cytometry. The expression of molecule on homing receptor(α_4β_7, L-selectin, LFA-1) in lymph fluid was observed by RT-PCR. Results: 1 week after the operation, compared with those in sham operation group, the positive rate of Qihuang Decoction group and contrast group decreased significantly(P0.01). Compared with those in the contrast group, the positive rate of Qihuang decoction group increased, but showed insignificant difference between the two groups(P0.05).Compared with those in sham operation group, the gene expression of Qihuang decoction group and contrast group decreased significantly(P〈0.01). Compared with those in the contrast group, Qihuang decoction group increased significantly(P 0.05). Additionally, the number of LFA-1 molecule expression was much more than α_4β_7 and L-selectin significantly. Conclusions: Qihuang Decoction can promote the positive rate and the gene expression of lymphocyte homing receptors of α_4β_7,
