摘 要:目的研究新生儿脐血单核细胞(MONO)和中性粒细胞(PMN)在胎膜早破时的变化及意义,探索早期新生儿感染的预测方法。方法应用流式细胞术检测45例足月胎膜早破新生儿脐血MONO和PMN表面TREM-1及CD64的水平,以同期正常新生儿45例为对照,比较胎膜早破组和健康对照组、胎膜早破感染组和胎膜早破无感染组之间的差异。结果 1胎膜早破组脐血MONO和PMN表面TREM-1、CD64均显著高于对照组,且差异有显著性(P<0.05);2胎膜早破感染组脐血MONO和PMN表面TREM-1、CD64均显著高于胎膜早破无感染组(P<0.05);3在评估早期新生儿感染时,脐血MONO和PMN表面TREM-1具有显著正相关性(r=0.768,P<0.05);脐血MONO和PMN表面CD64亦具有显著正相关性(r=0.667,P<0.05)。结论脐血MONO和PMN表面TREM-1及CD64的表达与胎膜早破及新生儿感染有关,临床检测有助于早期新生儿感染的预测。Objective To investigate early prediction effects for neonatal infection through detecting monocyte( MONO) and polymorphonuclear neutrophil granulocytes( PMN) TREM-1 and CD64 of umbilical blood with PROM.Method The levels of MONO and PMNTREM-1 and CD64 in umbilical blood of 45 vaginal delivery newborns with PROM 24 hours( PROM group) and and 45 normal ones( healthycontrol group) were detected with the application of flow cytometry. Compare the difference between PROM group and healthy control group,and the differences between infection group and non infection group. Result 1The levels of umbilical blood MONO and PMN TREM-1,CD64 in PROM group were higher than those of control group( P 〈0. 05); 2The levels of umbilical blood MONO and PMN TREM-1,CD64 in neonatal infection group were higher than those of non infectious group with PROM( P 〈0. 05); 3When assessing neonatalinfection,umbilical blood MONO has a significant positive correlation with PMN( r = 0. 768,P〈 0. 05); Umbilical blood MONO CD64 has a significant positive correlation with PMN( r = 0. 667,P 〈0. 05). Conclusion Changes of umbilical blood MONO and PMN TREM-1 and CD64 are associated with neonatal infection,joint detection helps to early prediction of neonatal infection.
