负压封闭引流技术在儿童浅Ⅱ度烧烫伤创面临床应用

摘　　要：目的探讨负压封闭引流技术（VSD）治疗儿童浅Ⅱ度烧、烫伤创面的临床应用效果。方法选取沈阳军区总医院自2012年1月至2015年5月收治的48例浅Ⅱ度烧烫伤患儿。共68个创面,单个患儿创面面积〈15%。将48例患儿随机分为对照组与研究组,每组各24例。对照组32个创面,予以常规换药治疗,创面碘伏清洗,外用表皮生长因子等包扎,根据渗出情况1~2 d换药1次;研究组36个创面,入院后24 h内创面予以负压封闭引流套装行持续负压吸引治疗,压力0.02~0.06 kPa之间,1周后拆开负压装置。比较两组创面换药次数、愈合时间。结果两组创面经过不同处理后均达到愈合目的,研究组的换药次数和愈合时间均少于对照组,两组间比较差异有统计学意义（P〈0.05）。结论 VSD治疗可明显减少创面换药次数,加快愈合时间,并极大的减轻了患儿换药时的痛苦,适用于小儿浅Ⅱ度烧烫伤的治疗。Objective To explore the curative effect of vacuum sealing drainage（ VSD） on the repair and reconstruction of superficial Degree Ⅱ burn and scald wound. Methods Totally 48 children with 68 burn or scald wounds of superficial Degree Ⅱ（ area 15%） were enrolled in this study from January 2012 to May 2015. All 48 cases were divided into experimental group and control group,with 24 cases in each group. Wounds in the control group were treated by conventional therapy,which were cleaned by iodophor and bandaged by epidermal growth factor,and dressing changed at 1 or 2 days according to exudation condition. While,wounds in the experimental group were treated by VSD and the pressure was kept between 0. 02 and 0. 06 k Pa. VSD device was removed after 1week,and the dressing change times and healing time were compared between the two groups. Results All cases in both groups healed postoperatively. While,the dressing change times in cases of the experimental group were significantly less than those of the control group,and healing time was also shorter in the experimental group when compared to the control gourp. Conclusion VSD is adapted to the clinical application on the repair of children＇s superficial Degree Ⅱ burn and scald wounds,which can reduce the dressing change times and healing time.
