小剂量环孢素A、司坦唑醇联合补肾中药治疗慢性再生障碍性贫血的疗效研究

摘　　要：目的:观察小剂量环孢素A、小剂量司坦唑醇联合补肾中药治疗慢性再生障碍性贫血的疗效。方法:选择2005年1月-2014年6月在我院就诊及住院的慢性再生障碍性贫血患者90例,随机分为中西医结合治疗组(46例)和西医治疗组(44例)。西医治疗组:给予环孢素A 3 mg·kg-1·d-1,分2次口服,司坦唑醇6mg/d,分3次口服,14岁以下4mg/d,分2次口服。中西医结合组:在西医治疗组的基础上根据中医辨证分型:肾阴虚型、肾阳虚型、肾阴阳两虚型给予不同方剂治疗,治疗6个月后评价疗效。结果:中西医结合组和西医治疗组的有效率分别为85%(39/46)和82%(36/44),两组相比差异无统计学意义(P>0.05)。两组肝、肾损害的发生率分别为21.7%(10/46)、2.2%(1/46),40.9%(18/44)、13.6%(6/44),两组相比具有统计学差异(P<0.05)。结论:小剂量环孢素A、小剂量激素治疗慢性再障能达到常规剂量的疗效,节约了资源,降低了副作用的发生率。在此基础上联合应用补肾中药,进一步降低了副作用的发生率,提高了患者的耐受性,且疗效稳固。Objective： To observe clinical effects of small dose of cyclosporine A plus small dose of stanozolol and TCM medicine on chronic aplastic anemia. Methods： 90 patients from 2005 January to 2014 June in our hospital were randomly divided into the combined treatment group （46 cases） and the western medicine group （44 cases）. Cyclosporine was given to patients in the western medicine group. The other group received different medicine according to the TCM dialectical type. In 6 months, clinical efficacy was evaluated. Results： The efficiencies in two groups were 85% （39/46） and 82% （36/44） respectively, with no significant difference （P〉0.05）. Compromised incidence in liver and kidney in two groups were 21.7% （10/46）, 2.2% （1/46）, and 40.9% （18/44）, 13.6% （6/44） respectively, with significant difference （P〈0.05）. Conclusion： Treating chronic aplastic anemia with small dose of cyclosporine A plus small dose of stanozolol could save resources, reduce the incidence of side effects as conventional dose. The integrative medicine could further reduce the incidence of side effects, and improve the tolerance, with stable curative effects.
