宫腔镜诊治131例宫内节育器嵌顿患者的临床分析

摘　　要：目的分析宫内节育器（IUD）嵌顿的发生原因.方法：回顾性分析宫腔镜诊治的131 例IUD 嵌顿患者,根据居住地不同分为A 组和B 组,锦州市内患者60 例,为A 组,锦州市周边地区患者71 例,为B 组.从患者居住地、年龄、带器时间及节育器种类等探讨宫内节育器嵌顿发生的的原因.结果：B 组患者因外院取环失败就诊明显高于A 组患者,同时B 组患者绝经1 年后取环比率明显高于A 组患者,两组比较有统计学意义（P〈0. 05）,A 组患者取出的嵌顿节育器中爱母环所占比例高于B 组,而金属圆环所占比例明显低于B 组患者,两组比较有统计学意义（P〈0. 05）,发生节育器嵌顿时IUD 置入时间为1 -30 年,中位数为10. 0 年,约60%的A 组患者发生节育器嵌顿在放置IUD 后的9年内,而60%B 组患者在放置IUD 后12 年内,A 组患者和B 组患者发生节育器嵌顿时间分别8. 0（4. 0 -11. 0）年和12. 0（4. 0 -18. 0）年,两组差别有统计学意义（Zc=2. 11,P =0. 001）.结论：宫内节育器嵌顿发生与带器者年龄,带器时间有关,也与带器者居住地区有关.Objective:To study causes of incarceration of intrauterine device ( IUD) . Methods:The clinical data of 131 ca-ses diagnosed with incarcerated IUD by hysteroscopy was studied retrospectively about place of residence of patients, age, time of using IUD and types of incarceration of IUD. Results:The residents of the surrounding areas of Jinzhou had significantly higher rate of out-of-hospital IUD extraction failure and IUD extraction rate 1 year after menopause than the Jinzhou city residents, and the differences were statistically significant (P<0. 05). Among the extracted IUD, there were more Mcu IUDs and fewer metal IUD of the Jinzhou city residents than those of the residents of the surrounding areas of Jinzhou, and the differences were statistically significant (P<0. 05). When the incarceration occurred, the IUD insertion time was 1-30 years with a median of 10. 0 years (9 years for 60% of the Jinzhou city residents, while 12. 0 years for 60% of the residents of the surrounding areas of Jinzhou). The time of incarceration of ICU of the Jinzhou city residents and the residents of the surrounding areas of Jinzhou was 8. 0 (4. 0~11. 0) and 12. 0 (4. 0~18. 0) years, and the difference had the statistical significance (Zc=2. 11, P=0. 001). Conclusions:The occurrence of the incarceration of IUD is re-lated with the age of the woman with IUD, the time of IUD placement and the residence place of the woman with IUD.
