慢性肾功能不全血液透析患者血尿酸水平与高血压的关系

摘　　要：目的探讨慢性肾功能不全行血液透析患者血清尿酸及相关指标与高血压的关系。方法选取2013年5—10月天津医科大学第二医院100例慢性肾功能不全行血液透析的患者,以血尿酸正常值(440μmol/L)为参考标准,将100例患者分为血尿酸升高组和血尿酸正常组,同时测定钙,钾,无机磷,肌酐,白蛋白,血压等指标;另选取5例高尿酸并发高血压的血液透析患者给予别嘌呤醇100mg,1次/d,口服,4周后测定血尿酸和血压,进行回顾性研究。结果尿酸升高组高血压的发生率为61.9%,尿酸正常组高血压的发生率为20.7%,二组间χ2值为17.56,差异有统计学意义(P<0.05),其余指标差异无统计学意义(P>0.05);5例高血压合并高尿酸患者给予治疗后,血压和血尿酸显著降低(P<0.05)。结论慢性肾功能不全血液透析患者的血尿酸与高血压互有影响,控制血尿酸可能有助于透析患者血压的控制。Objective To investigate the relationship between serum uric acid with the related indicators and hypertension in chronic renal insufficiency patients with hemodialysis.Methods One hundred cases of chronic renal insufficiency patients with hemodialysis were selected from author＇s hospital and divided into two groups,high serum uric acid group and normal group,according to the normal serum uric acid（430μmol/L）.And calcium,potassium,phosphorus inorganic,creatinine,albumin,blood urea nitrogen and blood pressure were also measured.Five cases of hemodialysis patients with high uric acid and hypertension were chosen for the retrospective study with the treatment of allopurinol 100 mg orally once a day,and uric acid and blood pressure were measured after 4 weeks.Results The incidence of hypertension in the high uric acid group was 61.9%,and in the normal uric acid group was 20.7%,the difference between the both was statistically significant（P〈0.05）.There were no significant statistically differences in comparison between other test items（P〉0.05）;After the treatment,the blood pressure and serum blood uric acid of five cases of patients with hypertension and serum uric acid decreased significantly（P〈0.05）.Conclusions Hypertension and serum uric acid in dialysis patients influence each other,and controlling the serum uric acid of dialysis patients may help control blood pressure in dialysis patients.
