摘 要:目的分析该院门诊药物咨询室用药咨询案例,探讨这一新型药学服务模式在医院的实践。方法对该院2013年4—12月药物咨询室的咨询记录进行回顾性分析。结果咨询者身份以患者为主;用药咨询人员以老年人(>60)为主.患者咨询内容范围较广,涉及7个方面,药品适应证及用法用量、不良反应及注意事项、药物相互作用的咨询比例位列前三,分别为37.82%、26.94%、13.47%。咨询药物类型以骨科用药、心脑血管系统药物、内分泌系统药物为主。96.37%的患者得到现场解答。咨询难点以患者用药情况复杂为主,占71.43%。咨询问题解决方式以说明书为主,占74.48%。结论药物咨询室这种以患者为中心的新型医疗服务模式提高了药学服务质量。在促进了患者安全、有效、经济且合理的使用药物的同时也提高了药师的知识水平及沟通能力。Objective To explore the practice of a new pharmaceutical care model in hospitol by analyzing cases in outpatient drug consultation room.Methods A retrospectivelly analysis was performed by studying the consultation records in the room between April and December in 2013.Results The consultants were mainly comprised of patients over 60 years old.And the consulted questions were quite wide,reached to seven classifieds,such as drug indications and usage,adverse drug reactions and annoucements,and drug interactions,which occupied the high percentages of 37.82%,26.94%,13.47% respectively.The drugs for orthopedics,cardio-cerebrovascular and endocrine systems were most fequently consulted.96.37% patients obtained their explanations at once.The difficult consultant point was mainly the complexity of patients' drugs use acounting for 71.43%.Advisory problem solving approach to the specification was most of 74.48%.Conclusion Drug counseling room of this new patient-centered care model improves the quality of pharmaceutical care.It not only promotes safe,effective,economical and rational use of drugs in patients,but also improves the level of knowledge and communication skills of pharmacists.
