摘  要:目的:评价前路减压治疗颈椎骨质疏松性骨折的早期临床疗效及其对血清中基质金属蛋白酶-3(MMP-3)和白细胞介素-6(IL-6)表达水平的影响。方法:对48例颈椎骨质疏松性骨折采用前路减压手术治疗,术前、术后JOA评分、Cobb’s角测量评价临床疗效,采取静脉血检测术前、术后的血清MMP-3与IL-6水平。结果:治疗预后优良率为91.7%,术后JOA评分与伤椎高度比显著升高,Cobb角和椎管占位率明显下降,术后的血清MMP-3与IL-6值都明显降低,手术前后对比差异有统计学意义(P<0.05)。术后随访6个月,浅表感染、内固定断裂及颈椎不稳等并发症发生率为4.16%。结论:前路减压治疗颈椎骨质疏松性骨折的预后效果佳,能充分减压,迅速恢复颈椎稳定性,改善脊柱功能,安全性高,其有效降低血清MMP-3与IL-6水平可能是其发挥临床疗效的机制之一。Objective: To investigate the efficacy of anterior decompression on osteoporotic cervical frac- tures and its effect on inflammatory cytokines. Methods: A total of 48 cases of cervical osteopo- rotic fractures were treated with anterior decompression. The clinical efficacy was evaluated by JOA scores, height ratio of the injured vertebral body, Cobb angle, canal volume occupying rati- o,serum levels of MMP-3 and IL 6. Results: The rate of excellent prognosis in the treatment group was 91. 7~. JOA scores and height ratio of the injured vertebral body significantly in- creased, while the Cobb angle and canal volume occupying ratio, MMP-3 and IL-6 levels in the serum significantly decreased (P〈0.05). During the six-month follow-up, the rate of complica- tions such as superficial infection, screw breakage and cervical instability was 4.16%. Conclusion: The effect of anterior decompression on osteoporotic cervical fractures and inflamma- tory cytokines is positive with complete decompression, reduction of cervical stability and impro- ving spinal function. Effectively reducing the serum levels of MMP-3 and IL-6 may be one of the mechanisms of its clinical effectiveness.
