摘  要:目的研究脑外伤患者血清中巨噬细胞炎症蛋白-1α(MIP-1α)的动态变化及临床意义。方法利用ELISA方法对2012年12月~2014年6月收治在安康市中心医院神经外科的55例脑外伤患者(轻型组18例,中型组20例,重型组17例)入院后第1、3、7天的血清MIP-1α水平进行动态监测,并与对照组15例进行比较分析。结果入院后第1、3天时,脑外伤轻、中、重型组患者血清MIP-1α水平均显著高于对照组,且三组中重型组MIP-1α水平最高,中型组次之,轻型组最低(P〈0.01)。入院第7天时,各脑外伤患者组血清MIP-1α的表达水平均显著下降,其中,中型及重型组患者的血清MIP-1α表达水平高于对照组(P〈0.05),而轻型组患者的血清MIP-1α表达水平与对照组比较差异无统计学意义(P〉0.05)。入院后第1、3、7天,脑外伤患者血清中的MIP-1α水平与格拉斯哥昏迷评分(GCS)呈负相关,受伤程度越重,MIP-1α水平越高(第1天:r=-0.703,P〈0.01;第3天:r=-0.785,P〈0.01;第7天:r=-0.518,P〈0.01)。结论脑外伤后MIP-1α在血清中可被检出,且在伤后第3天表达最高,动态监测MIP-1在判断患者病情严重程度及预后中起重要作用。Objective To explore the dynamic changes of serum MIP-1α in patients after traumatic injury(TBI) and its clinical significance. Methods The serum MIP-1α levels of 55 cases, who suffered traumatic brain injury treated in the Neurosurgery Department, Center Hospital of Ankang City from December 2012 to June 2014(18 cases in the mild group, 20 cases in the moderate group, 17 cases in the severe group) were dynamic monitored by ELISA at 1st, 3rd, 7th d after admission, and compared with 15 healthy individuals in the normal control group. Results The serum levels of MIP-1α in the brain injury group were significantly higher than those in the normal control group at the 1st and 3rd day; the serum level of MIP-1α was highest in the severe group and lowest in the mild group among the three groups(all P〈0.01). And at the 7th day, the serum levels of MIP-1α in TBI group had significantly dropped, MIP-1α levels in the moderate group and the severe group were still higher than those in the normal control group(P〈0.05), but there was no statistically significant difference in serum MIP-1α levels between the mild group and the normal control group(P〈0.05). MIP-1α levels of TBI patients at each time point was negatively correlated with Glasgow coma score(GCS), the more severe degree of TBI, the higher level of MIP-1α(1st day: r =-0.703, P〈0.01; 3rd day: r =-0.785, P〈0.01; 7th d:r =-0.518, P〈0.01). Conclusion MIP-1α can be detected in serum after TBI and its expression significantly increased, peaked at 3rd day. The dynamic monitoring of the serum MIP-1α levels in the patients with traumatic brain injury plays an important role in judging the disease severity and prognosis.
