摘  要:目的:运用机械振动力学数值计算方法,比较三种腰椎侧卧调整手法[传统腰椎侧卧调整手法(传统斜扳法)、改良腰椎侧卧调整手法(改良斜扳法)与特异性腰椎侧卧调整手法]杠杆力加载的力学特征。方法:由具有20年以上临床经验的推拿科医生按标准操作三种手法,观察定性后,假设椎体为刚体,周围组织为黏弹性体,建立模拟L1-L5的五自由度机械振动力学模型,模拟加载不同施力模式,对基本方程进行求解。结果:载荷相同的条件下,在频率≤1的特异性调整手法作用下,同位椎体的最大位移都大于传统及改良调整手法作用下的椎体最大位移,且目标椎体的位置越低,损伤比也越低。结论:椎体位移相同(非0)的条件下,运用简谐激励的特异性调整手法比脉冲激励的传统及改良调整手法所需加载力更小,损伤风险更低。Objective: To compare the mechanical characteristics of lever force loadings among three different kinds of lumbar lateral posture manipulations that contains the traditional lumbar lateral adjustment technique,the improved lumbar lateral adjustment technique and the specific lumbar lateral adjustment technique applying numerical calculation method for mechanical vibration. Methods: All three manipulations were operated standardly by the doctors with clinical experience over20 years. A five dimensional mechanical vibration force model that simulates different manipulative force exerted on from L1 to L5vertebral bodies was built based on the hypothesis in which a vertebrae was taken as a rigid body and its surrounding tissue was taken as a viscoelastic body after qualitative observation,and finally the fundamental equations were solved. Results:Under the same force load conditions,one same vertebral body 's maximum displacement under specific lumbar lateral adjustment technique whose frequency is no more than 1,is greater than the displacement under traditional lumbar lateral adjustment technique and the improved lumbar lateral adjustment technique,and the lower position a vertebrae is targeted,the less damage ratio it gets. Conclusion: With the same vertebral spatial displacement( the numeral value is not 0),the load force of the specific lumbar lateral adjustment technique is smaller than the traditional lumbar lateral adjustment technique and the improved lumbar lateral adjustment technique,so does the risk of injury.
