奚氏针法对类风湿关节炎患者外周血单个核细胞中miR-146a表达的影响

摘　　要：目的:探讨奚氏针法治疗对类风湿关节炎(rheumatoid arthritis,RA)患者外周血单个核细胞(peripheral blood mononuclear cell,PBMC)中miR-146a表达的影响。方法:纳入60例RA患者,随机分为治疗组和对照组,每组30例。所有患者均给予西医常规治疗(泼尼松10 mg/d+甲氨蝶呤10 mg/周),治疗组患者辅以奚氏针法治疗,治疗周期为12周。检测并比较两组患者的PBMC中miR-146a的表达及外周血C反应蛋白(C-reactive protein,CRP)、血沉(erythrocyte sedimentation rate,ESR)水平。结果:治疗后,两组患者的PBMC中miR-146a相对表达量较治疗前均明显升高(P<0.01),且治疗组患者的PBMC中miR-146a的相对表达量高于对照组(P<0.05)。治疗后,两组患者的CRP及ESR水平较治疗前均明显降低(P<0.01),且治疗组患者的CRP及ESR水平低于对照组(P<0.05)。结论:奚氏针法结合西医常规疗法治疗RA,可明显降低RA的活动度,其机制可能与上调miR-146a表达相关。Objective： To investigate the effect of＂Xi＇s Acupuncture＂on the expression of miR-146 a in the peripheral blood mononuclear cells（ PBMC） of patients with rheumatoid arthritis（ RA）. Methods： A total of 60 patients with RA were selected and randomly divided into treatment group and control group with 30 cases in each group. All the patients were treated with conventional western medical therapy [prednisone（ 10 mg / d） and methotrexate（ 10 mg / w） ],and the patients in the treatment group were treated with ＂Xi＇s Acupuncture＂additionally. The treatment course was 12 weeks. The expression of miR-146 a in PBMC,the level of C-reactive protein（ CRP） in serum and the erythrocyte sedimentation rate（ ESR） were detected and compared. Results： After treatment,the expression of miR-146 a in PBMC in both groups was significantly increased（ P〈0. 01）,and the expression of miR-146 a in PBMC of patients in the treatment group was higher than that in the control group（ P〈0. 05）. After treatment,the levels of CRP and ESR in both groups were obviously decreased（ P〈0. 01）,and the levels of CRP and ESR in the treatment group were lower than those in the control group（ P〈0. 05）. Conclusion：＂Xi＇s Acupuncture＂combined with conventional western medical therapy can reduce the activity of RA and its mechanisms may have relationship with up-regulation of miR-146 a expression.
