摘 要:目的研究Bcl-2基因多态性、IL-21单核苷酸多态性与系统性红斑狼疮易感性的相关性。方法随机选取2012年5月~2015年5月间在我院接受治疗的200例系统性红斑狼疮患者作为研究对象,将其分为实验组;选取健康研究对象200例,分为对照组。观察两组研究对象Bcl-2主要基因型分布、等位基因分布以及IL-21单核苷酸多态性rs2243188位点等位基因及其基因型分布与系统性红斑狼疮临床症状的关系和Bcl-2与IL-21协同作用分析。结果两组研究对象Bcl-2主要基因分布差异明显,部分基因类型差异具有统计学意义(P〈0.05);两组研究对象Bcl-2常见等位基因分布差异明显,部分基因类型差异具有统计学意义(P〈0.05);IL-21单核苷酸多态性rs2243188位点等位基因及其基因型分布与系统性红斑狼疮临床症状密切相关;同时携带Bcl-2 195 bp和IL-21 138 bp易感等位基因的个体较单独携带Bcl-2 195 bp或IL-21 138 bp易感等位基因的个体患病的风险性更高。结论 Bcl-2基因多态性、IL-21单核苷酸多态性与系统性红斑狼疮发病密切相关,致病原因可能为两者直接参与系统性红斑狼疮发病部位或附近某个易感位点存在连锁不平衡,两者易感等位基因分布在决定系统性红斑狼疮易感性中存在协同作用。Objective To investigate the correlation of Bcl-2 gene polymorphism, IL-21 single nucleotide polymorphism and susceptibility of systemic lupus erythematosus. Methods Randomly selected 200 patients with systemic lupus erythematosus who received treatment in our hospital from May 2012 to May 2015 were randomly selected and divided into the experimental group, then 200 healthy subjects were selected and divided into the control group. The relationship between Bcl-2 major genotypes distribution, allele distribution, IL-21 single nucleotide polymorphism rs2243188 locus and its genotypes distribution and clinical symptoms of systemic lupus erythematosus were observed,the relationship between which and synergistic effect between Bcl-2 and IL-21 was analyzed. Results There were significant differences in the distribution of major genes of Bcl-2 in the two groups of study subjects, and the differences of partial gene types were statistically significant(P〈0.05). There were significant differences in the distribution of common alleles between the groups of research subjects, and the differences of partial gene types were statistically significant(P〈0.05). The allele gene of IL-21 single nucleotide polymorphism rs2243188 locus and its genotype distribution were closely related to the clinical symptoms of systemic lupus erythematosus. The risk of disease of people who carried with 195 bp Bcl-2 and 138 bp IL-21 susceptible allele genes was higher than those who carried 195 bp Bcl-2 or 138 bp IL-21 allele gene alone. Conclusion Bcl-2 gene polymorphism, IL-21 single nucleotide polymorphism are closely related to systemic lupus erythematosus, and the cause of the disease may be that both the two directly involved in the pathogenesis of systemic lupus erythematosus or there is a linkage disequilibrium in the vicinity of some susceptible locus.
