腹腔镜与开腹胃十二指肠溃疡穿孔修补术的临床疗效比较研究

摘　　要：目的通过对比分析腹腔镜胃十二指肠溃疡穿孔修补术（1aparoscopic neoplasty for perforated gastroduodenal ulcer,LNPGU）与传统开腹胃十二指肠溃疡穿孔修补术（open neoplasty for perforated gastroduodenal ulcer,ONPGU）的疗效,旨在为临床合理选择治疗方案提供科学依据。方法选取2014年1月～2015年1月间行腹腔镜胃十二指肠溃疡穿孔修补术治疗的胃十二指肠溃疡穿孔患者32例,同时选择2012年12月～2013年12月行传统开腹胃十二指肠溃疡穿孔修补术患者32例作为对照。观察并对比两组的手术时间、术中出血量、术后肠鸣音恢复情况、住院时间及并发症发生率。结果 LNPGU组的术中出血量显著少于ONPGU组,LNPGU组的肠鸣音恢复时间、住院时间显著短于ONPGU组,差异有统计学意义（P〈0.05）;LNPGU组并发症发生率显著低于ONPGU组,差异有统计学意义（P〈0.05）。结论腹腔镜胃十二指肠溃疡穿孔修补术的临床疗效在促进胃肠功能恢复、并发症方面显著优于传统开腹胃十二指肠溃疡穿孔修补术,值得推广和应用。Objective To compare the therapeutic effect of LNPGU and ONPGU,and to provide scientific basis for clinical rational choice of treatment plan. Methods 32 patients with perforated peptic ulcer perforation were selected from January 2014 to January 2015,and 32 cases were treated with traditional open gastric duodenal ulcer perforation repair from December 2012 to December 2013 as the control. The operation time, the amount of bleeding,recovery of bowel sounds,hospitalization time and the incidence of complications were compared between two groups. Results The amount of bleeding of LNPGU group patients was significantly less than that in the ONPGU group,the gurgling sound recovery time and the hospitalization time of LNPGU group was significantly shorter than that of the ONPGU group,the difference was significant（P〈0.05）.The incidence of complications in LNPGU group was significantly lower than that in ONPGU group（P〈0.05）. Conclusion The clinical efficacy of laparoscopic gastric and duodenal ulcer perforation repair in promoting gastrointestinal function recovery and complications are significantly better than the traditional open gastric duodenal ulcer perforation repair, it is worthy of promotion and application.
