摘　　要：目的探讨急性脑梗死中医临床路径的设计和应用评价。方法在西医治疗指南和中医最佳诊疗方案的基础上,建立急性脑梗死中医临床路径,2013年6月～2015年5月采用非同期对照研究,根据临床路径实施前后分为对照组和实验组,观察比较两组患者的平均住院日、人均住院费用,并记录两组患者在入院及治疗7 d、15 d、30 d、60 d的美国卫生研究院卒中量表（NIHSS）评分、日常生活能力评定（Barthel指数）。结果与对照组相比,实验组的平均住院日、人均住院费用较对照组显著降低（P〈0.01）,NIHSS评分和Barthel指数评定差异有高度统计学意义（P〈0.01）。结论急性脑梗死患者应用中医临床路径能有效保障临床疗效,缩短住院时间,控制住院费用,充分体现出中医临床路径在急性脑梗死医院管理和质量控制方面的价值。Objective To design and evaluate the effectiveness of clinical pathway of traditional Chinese medicine on acute cerebral infarction. Methods The clinical pathway of traditional Chinese medicine on acute cerebral infarction was developed based on the optimum treatment guidelines of traditional Chinese medicine（TCM） and western medicine.The contrast study from June 2013 to May 2015 was adopted, and the participants were randomly divided into trial and control groups. The length of hospital stay and expenses as per National Institution of Health Stroke Scale and daily life ability assessment（Barthel index）, at 7,15,30,and 60 days. were observed and compared between the two groups. Results The length of stay and expenses in the trial group were significantly lower than the control group（P〈0.01）. The difference between two groups after analyzing NIHSS scores and Barthel index was statistically significant（P〈0.05）. Conclusion The application of TCM clinical pathway on patients with acute cerebral infarction significantly improves the outcome of treatment, shortens inpatient stay, and controls hospital expenses. It fully reflects the effectiveness of the traditional Chinese medicine clinical pathway on acute cerebral infarction and the value of hospital management.
