儿童膀胱过度活动症的发病危险因素及综合治疗效果研究

摘　　要：目的:探讨儿童膀胱过度活动症(OAB)的发病危险因素及其综合治疗的效果。方法:收集2009年1月-2013年12月在济南市儿童遗尿门诊就诊的1 110例OAB儿童,仔细询问病史,进行微量元素、腰骶椎平片、尿常规、泌尿系超声等相关检查。然后将其随机分为超短波耳穴组、基础治疗组、补锌组、奥昔布宁组、综合治疗组、无治疗对照组,给与相应的治疗,治疗前后均做OAB症状评分(OABSS)。结果:OAB发病主要危险因素有:腰骶椎隐裂、微量元素锌缺乏、恐惧、不当饮食、玩电子游戏等相关。5个治疗组治疗后OABSS得分均较本组治疗前明显降低(P<0.01);治疗后6个组之间OABSS得分的差异有显著统计学意义(F=33.2,P<0.01)。结论:隐性脊柱裂、微量元素锌缺乏、受到恐惧、饮食不当、玩电子游戏等情况可能诱发儿童OAB,避免上述因素可以有效预防OAB的发病。基础治疗加超短波耳穴加补锌治疗的综合治疗效果最优,且简便易行,具有更好的安全性和依从性,是治疗小儿OAB更为理想的方法,在临床上应广泛推广应用。Objective：To explore the risk factors and comprehensive treatment effect of overactive bladder（OAB） in children.Methods：One thousand one hundred and ten cases of children with OAB of outpatients from January 2009 to December2013 were included.The medical history of patients was inquired carefully,and the microelements,lumbosacral vertebral X-ray,urine routines and ultrasound of urinary system were all tested.Then the patients were randomly divided into super short wave ear acupoint group,basic treatment group,zinc supplementation group,oxybutynin group,comprehensive treatment group,and control group.Patients in the groups were treated with corresponding therapy.The symptom score of OAB patients before and after treatment was evaluated.Results：The risk factors for occurrence of OAB included cracked lumbosacral vertebral,zinc deficiency,fear,improper diet,playing video games and so on.The symptom score of OAB patients in treatment groups after treatment decreased significantly compared with that of treatment before（P〈0.01）.After treatment,the difference in symptom score of OAB patients among the 6 groups was significant（F=33.2,P〈0.01）,the score of patients in comprehensive treatment group was the lowest,and the curative effect was the best of all.Conclusion：cracked lumbosacral vertebral,zinc deficiency,fear,improper diet and playing video games might induce OAB in children,and to avoid the above factors could effectively prevent the occurrence of OAB.The curative effect of comprehensive treatment as combination of super short wave ear acupoint and zinc supplementation was the best,which as an ideal therapy for OAB in children was simple and security,and worth to be popularized and applied in clinic.
