急性脑出血大鼠应激性肝损害细胞凋亡的变化及涤痰通瘀方的干预作用

摘　　要：目的:探讨急性脑出血大鼠模型下应激性肝损害细胞凋亡的变化及涤痰通瘀方对肝损害的保护作用。方法:选用健康雄性SD大鼠,随机分成正常对照组、脑出血组、涤痰通瘀组。采用大脑注射Ⅶ型胶原酶法建立大鼠急性脑出血模型,检测造模后24、48、72h各组大鼠肝功能水平及观察肝组织HE染色结果,同时Tunel法检测各组大鼠肝细胞凋亡指数。结果:急性脑出血大鼠模型下应激性肝损害各时相点均可见较多凋亡细胞,48h可见凋亡细胞达高峰,24、72h可见凋亡细胞减少,经涤痰通瘀方干预后,各时相点Tunel阳性细胞均显著减少,涤痰通瘀组大鼠肝细胞凋亡数虽较正常对照组显著增多(P<0.01),但较脑出血组显著降低(P<0.05,P<0.01)。同时与脑出血组相比,涤痰通瘀方可显著降低血清ALT、AST水平(P<0.05,P<0.01)。结论:急性脑出血大鼠模型可导致应激性肝损害细胞凋亡,经涤痰通瘀方干预后,可以有效抑制肝细胞凋亡,改善肝组织病理学结构,提高肝功能水平,是防治脑出血应激性肝损害的重要途径。Objective：To study the changes in cell apoptosis caused by hepatic stress injury in rats with acute cerebral hemorrhage and protective effect of Ditan Tongyu Decoction on hepatic injury.Methods：Healthy male SD rats were randomly divided into control group,cerebral hemorrhage group and Ditan Tongyu group.The rat models of acute cerebral hemorrhage were established by injecting VII collagenase in brain.Hepatic function and Hepatic tissue pathology were observed in 24 h,48h and 72 h after model establishing.The apoptosis indexes of hepatic cells were tested by using Tunel method.Results：There were many apoptotic cells during each phase of hepatic stress injury rats with acute cerebral hemorrhage,which reached to its peak within 48 h.After intervening with Ditan Tongyu Decoction,the number of Tunel positive cells reduced remarkably,and the apoptotic cells of rats in Ditan Tongyu Decoction group was more than that of control group（P〈0.01）.The difference in apoptotic cell numbers between control group and cerebral hemorrhage group,and Ditan Tongyu Decoction group and cerebral hemorrhage group was also significant（P〈0.05,P〈0.01）.Compared with cerebral hemorrhage group,the levels of ALT and AST in serum of rats in Ditan Tongyu Decoction group decreased significantly（P〈0.05,P〈0.01）.Conclusion：Acute cerebral hemorrhage could lead to irritability hepatocyte apoptosis in rats,and intervention of Ditan Tongyu Decoction could inhibit hepatocyte apoptosis,improve pathology changes,and increase the hepatic function effectively,which is an important way for preventing and treating hepatic stress injury caused by acute cerebral hemorrhage.
