阴道用乳杆菌活菌胶囊联合复方黄柏液促进子宫全切术后阴道残端愈合的临床分析

摘　　要：目的探讨阴道用乳杆菌活菌胶囊(定君生)联合复方黄柏液外用对促进子宫全切术后阴道残端愈合的临床疗效。方法选取2014年因妇科良性、非炎症性疾病在四川省人民医院妇科实施子宫全切术的患者160例,分为研究组和对照组,每组各80例。研究组于术后10 d开始予以复方黄柏液稀释坐盆,1次/d,坐盆完毕后予乳杆菌活菌胶囊阴道内放置,1粒/d,连续10 d;对照组无用药。比较两组阴道残端愈合率、阴道残端出血率、阴道残端肉芽组织形成率、阴道清洁度异常率、阴道菌群失调检出率、细菌性阴道病的检出率。结果研究组甲级愈合率为88.75%,高于对照组的72.5%(P<0.05),研究组阴道残端出血率(1.25%)、阴道残端肉芽形成率(2.5%)、阴道清洁度异常率(27.5%)、阴道菌群失常检出率(18.75%)、细菌性阴道病的检出率(1.25%)均低于对照组(8.75%、11.25%、60.0%、33.75%、8.75%)(P<0.05)。结论阴道用乳杆菌活菌胶囊联合复方黄柏液用于子宫全切除术后可减轻阴道残端的炎症反应,调节阴道菌群,改善阴道环境,减少阴道炎症发生,促进阴道残端愈合。Objective To investigate the clinical effect of vaginal Lactobacillus live capsule combined with compound Phellodendron liquid in promoting the healing of vaginal stump after hysterectomy. Methods 160 patients underwent hysterectomy due to gynecological benign or non inflammatory diseases in Department of gynecology of Sichuan Provincial People’s Hospital from Jan 2014 to Dec 2014 were divided into research group and control group randomly,80 cases in each group. 10 days after the operation,the cases in research group were dealt with compound Phellodendron liquid dilution of the sitting basin,once a day,and then vaginal Lactobacillus live capsule was placed,1 tablet per day,this kind of treatment lasted for 10 days; the control group without drug.Compared vaginal stump healing rate,vaginal stump hemorrhage rate,vaginal stump granulation tissue formation rate,vaginal cleanliness abnormality rate,vaginal dysbacteriosis rate,bacterial vaginal disease rate of two groups. Results Class- A healing rate of research group was 88. 75 %,higher than that of control group（ 72. 5 %）,and the difference was statistically significant（ P 〈0. 05）. Vaginal stump hemorrhage rate,vaginal stump granulation formation rate,the abnormal rate of vaginal cleanliness,vaginal dysbacteriosis rate and bacterial vaginal disease rate of research group were lower than those of the control group,and the differences were statistically significant（ P 〈 0. 05）. Conclusion Vaginal Lactobacillus live capsule combined with compound Phellodendron liquid can reduce the inflammatory reaction of vaginal stump,regulate the vaginal flora,improve the vaginal environment,reduce the occurrence of vaginitis and promote the healing of vaginal stump.
