外周静脉镇痛泵用于子宫肌瘤介入手术后观察

摘　　要：目的探讨外周静脉镇痛泵在子宫肌瘤介入治疗中的止疼效果。方法选取2013年3月至2015年1月在我院行子宫动脉栓塞术的子宫肌瘤患者163例,随机分为观察组(采用静脉自控镇痛泵)和对照组(患者自觉疼痛时给予镇痛药物),观察记录两组患者术后48h镇痛效果,舒适程度、镇静状态评分、术后下床活动时间以及不良反应等。结果观察组镇痛有效率为84.52%,明显高于对照组的40.51%,差异比较有统计学意义(X^2=33.912,P<0.05)。观察组患者舒适程度评分和镇静状态评分均显著高于对照组,差异有统计学意义(t值分别为9.057、6.176,均P<0.05)。观察组患者术后下床活动时间和住院天数均显著短于对照组的,差异有统计学意义(t值分别为-15.663、-5.776,均P<0.05)。两组不良反应有恶心、呕吐、皮肤瘙痒,但比较无统计学差异(X^2=0.387,P>0.05)。结论外周静脉镇痛泵能有效缓解子宫肌瘤介入手术术后疼痛症状,且安全有效,值得在临床稚广使用。Objective To investigate pain relieving effect of peripheral intravenous analgesia pump on interventional treatment of uterine fibroids.Methods From March 2013 to January 2015, 163 uterine fibroids patients treated by uterine arterial embolization in Second Hospital Affiliated to Suzhou University were randomly divided into observation group （ treated with intravenous analgesia pump ） and control group （provided with analgesic drugs when perceiving pain ）.Postoperative 48h analgesic effect, comfort degree, sedation score, postoperative out-of-bed activity time and adverse reactions were observed and recorded for two groups .Results In the observation group analgesic effective rate was 84.52%, which was higher than that （40.51%） of the control group, and the difference was significant （χ2 =33.912,P〈0.05）.Comfort degree score and sedation score were significantly higher in the observation group than in the control group （t value was 9.057 and 6.176, respectively, both P〈0.05）.Compared with the control group , postoperative out-of-bed activity time and hospitalization stay were obviously shorter in the observation group （t value was -15.663 and -5.776, respectively, both P〈0.05）. Adverse reactions such as nausea , vomiting and pruritus were found in two groups , but the differences were not significant （χ2 =0.387, P〉0.05）.Conclusion Peripheral intravenous analgesia pump can effectively alleviate postoperative pain symptoms in patients with uterine fibroids.It is safe and effective, and it is worthy of using in clinical practice .
