摘 要:目的探讨白细胞介素1β(IL^(-1)β)-511C/T和+3954C/T与无症状慢性阻塞性肺疾病(无症状COPD)的相关性。方法收集2009年1月~2013年6月河北联合大学附属开滦总医院呼吸科门诊筛查的300例无症状COPD患者及320例同期健康对照人群,用酶联免疫吸附试验(ELISA)检测血清IL^(-1)β水平,应用聚合酶链反应-限制性片段长度多态性(PCR-RFLP)方法检测-511C/T、+3954C/T位点的基因型。结果无症状COPD组的血清IL^(-1)β水平为(3.93±0.44)ng·m L^(-1),对照组为(2.73±0.12)ng·m L^(-1),组间差异具有统计学意义(P<0.001)。无症状COPD组IL^(-1)β-511的CC、CT和TT基因型频率分别为20.0%、63.0%和17.0%,对照组分别为22.2%、60.9%和16.9%,2组相比差异无统计学意义(P=0.336)。无症状COPD组IL^(-1)β+3954CC、CT和TT基因型频率分别为84.0%、16.0%和0%,对照组分别为86.9%、12.8%和0.3%,2组相比差异无统计学意义(P=0.795)。2组人群IL^(-1)β-511、+3954等位基因频率比较差异无统计学意义(P=0.684和P=0.387),且男性和女性人群亚组分析中2组人群差异均无统计学意义(P>0.05)。结论无症状COPD组血清IL^(-1)β水平高于对照组;IL^(-1)β中-511、+3954的基因型频率与等位基因频率与无症状COPD的发生无明显相关性。Objective To explore the relationship between polymorphism of interleukin-1β (IL-1β) - 511C/T and + 3954C/T and non-symptomatic chronic obstructive pulmonary disease. Methods Totally 300 patients with non- symptomatic COPD were selected who visited the Outpatient Department ofKai Luan General Hospital from January 2009 to June 2013. Another 320 healthy controls were recruited in the Medical Examine Center. Data were obtained through physical examination and a unified questionnaire. Enzyme-linked immunosorbent assay was used to mea- sure the level of IL- 1β in the serum. Polymerize chain reaction-restriction fragment length polymorphism was used to detect the genotypes of IL-1β ( - 511, +3954). Multivariate logistic regression was used to explore the relation between risk factors for susceptibility of non-symptomatic COPD. Results The level of IL-1β in the serum was (3.93 ±0.44) ng · mL - 1 in the non-symptomatic COPD group, while that was (2.73 ± 0.12) ng ·mL 1 in the control group with statistical significance (P 〈 0.001) between the 2 groups. The genotype frequency of - 511 site was: CC-52 (20.0%), CT-164 (63.0%) and TT-44 (17.0%) in the non-symptomatic COPD group, CC-58 (22.2%), CT-158 (60.9%) and TT-45 (16.9%) in the control group, and no significance difference was found (P = 0.336). The genotype frequency of + 3954 site was: CC-221 (84.0%), CT-39 (16.0%) and TT-0 (0%) in the non-symptomatic COPD group; CC-231 (86.9%), CT-28 (12.8%) and TT-1 (0.3%) in the control group, and no significance difference was found ( P = 0.795). The genotype frequency of- 511 siteand + 3954 site in the 2 group no significance difference was found (P = 0.684, P = 0.387). Not only in the total but also in the subgroups of men and women, the difference in allele frequency in the two sites was not found.
