摘　　要：目的探讨人端粒酶逆转录酶(human telomerase reverse transcriptase,h TERT)在肺癌患者外周血微转移中的诊断价值。方法选取确诊为非小细胞肺癌(NSCLC)的患者40例,选择同期诊断为肺部良性疾病的患者20例为研究对象,在未进行任何治疗前收集各研究对象的外周血,实时荧光定量PCR方法检测血hTERT mRNA并进行评价分析。结果 1NSCLC患者外周血中h TERT mRNA相对表达量明显高于肺良性疾病患者(P<0.05);240例NSCLC患者外周血中hTERT mRNA的阳性表达率分别为58.2%,20例肺良性疾病患者h TERT mRNA阳性表达率为5%,明显低于肺癌组患者(均P<0.001);3经分析,患者的hTERT mRNA与肺癌分期(TNM)具有显著的关系(P=0.004),但是,患者的年龄、性别、吸烟、肿瘤的组织学类型及分化程度对h TERT mRNA的表达无明显相关性。结论外周血hTERT mRNA表达在肺癌细胞分子标志物的NSCLC临床诊断中具有潜在的应用价值。Objective To investigate the diagnostic value of human telomerase reverse transcriptase （hTERT） micro- metastases in peripheral blood of patients with lung cancer. Method The peripheral blood sample of 40 cases of NSCLC lung cancer, and 20 cases of benign lung lesions were collected before starting respective treatment, and the real-time PCR method was applied to detect and evaluate the hTERT mRNA. Result （1） The relative mRNA expression level of hTERT in NSCLC patients were significantly higher than those in benign lung disease patients （P〈0.05）. （2） The positive rate of mRNA expression of hTERT in peripheral blood from 40 patients with NSCLC were 58.2%, while that of hTERT expression in peripheral blood from patients with benign lung disease were 5%, which was markedly lower compared with NSCLC patients （P〈0.001）. （3） For patients with NSCLC, the tumor TNM stage was significantly correlated with positive rate of hTERT mRNA expression （P=0.004）, whereas there were no correlations between the positive rate of hTERT mRNA expression and age, gender, smoking, histopathlogical type and tumor cell differentiation degree. Conclusion The hTERT mRNA expressions in peripheral blood could be used as a molecular marker for diagnosis of NSCLC.
