Glidescope可视喉镜紧急气管插管80例临床分析

摘　　要：目的 观察Glidescope可视喉镜在麻醉科外急诊危重症患者气管插管术中的临床应用价值。方法 选取我院麻醉科外需紧急气管插管的急诊危重症患者80例,随机分为可视喉镜组（A组）和普通喉镜组（B组）各40例,分别观察两组气管插管时的声门暴露分级程度、声门暴露时间;一次插管成功率和插管时间;气管插管相关不良反应。结果 A组声门暴露分级Ⅰ~Ⅱ级39例（97.5%）,B组声门暴露分级Ⅰ~Ⅱ级28例（70%）;B组声门暴露平均时间（7.49±1.48）s明显长于A组（4.58±1.10）s,差异有统计学意义（P〈0.05）。B组一次性成功插管25例（62.5%）,A组一次性插管成功36例（90%）;B组平均插管时间（26.1±10.9）s明显长于A组（16.8±10.7）s,差异有统计学意义（P〈0.05）。B组呛咳、屏气、躁动及牙龈或口腔黏膜出血等不良反应明显高于A组（P〈0.05）。结论 Glidescope可视喉镜可提高麻醉科外急诊危重症患者气管插管术的成功率,缩短插管时间,用于急救插管具有较大的应用价值。Objective To investigate the application value of Glidescope video laryngoscope for difficult endotracheal in- tubation outside the operating room. Methods A total of 80 cases of difficult tracheal intubation patients outside the operating room were randomly and evenly divided into two groups （n=40 for each）. The A group was tracheal intubated with GIidescope, and the B group was tracheal intubated with normal laryngoscope. In group A and B, glottic exposure view, intubation time, success rate, postoperative complications were recorded. Results The Glottis exposure class Ⅰ-Ⅱ in group B were 28 cases （70%）, in group A were 39 cases （97.5%） ; Group B glottis exposure （7.49±1.48） s significantly longer than the average time, the glottis exposure average time of group A （4.58± 1.10） s, the difference was statistically significant （P〈0.05）. Suc- cessful intubation in group B was 25 cases （62.5%）, in group A was 36 cases （90%）, the average intubation time in Group B was （26.1±10.9） s significantly longer than the group A （16.8±10.7） s, the difference was statistically significant （P〈 0.05）. choke to cough, breathless, agitated, and bleeding gums or adverse reactions of oral mucosa bleeding in Group B was ob- viously higher than that in group A （P〈0.05）. Conclusion GlideScope visual laryngoscope outside the operating room can im- prove the success rate of emergency tracheal intubation, shorten the intubation time and has great application value.
