氟防龋材料、改性几丁质治疗口腔幽门螺杆菌感染及溃疡疗效观察

摘　　要：目的:观察氟防龋材料、改性几丁质生物胶对口腔幽门螺杆菌(Hp)感染人群的疗效和安全性。方法:618例(90 d观察有效病例598例)口腔Hp感染阳性,且无消化道症状的受试者随机分为两组。观察组310例(90 d观察有效病例302例),以非药物治疗牙膏联合氟防龋材料刷牙;其中82例口腔溃疡患者联合使用改性几丁质生物胶喷于口腔溃疡处。对照组308例(90 d观察有效病例296例),其中69例存在口腔溃疡,以空白实验为对照,每日晚餐后用非药物牙膏清洁口腔。比较两组患者治疗后各时间段Hp阳性率,观察药品不良反应发生情况。结果:消除口腔Hp方面,治疗30 d和90 d,观察组和对照组口腔Hp阳性率分别为53.55%、98.70%和7.94%、94.59%,差异有统计学意义(P<0.01)。治疗口腔溃疡方面,治疗7 d和30 d,观察组和对照组有效率分别为76.74%、15.19%和100%、32.91%,差异有统计学意义(P<0.01)。观察组在治疗过程中无严重不良反应发生。结论:氟防龋材料能明显减小口腔Hp感染阳性率。联合应用改性几丁质生物胶能缩短口腔溃疡的治愈时间,对控制口腔Hp感染具有重要价值。Objectlve：To investigate the therapeutic function and curative effect of fluorine methods material and modified chitin xanthan gnmin treatment of patients with oral helicobacter pylori （Hp） infection. Methods ：618 patients with oral Hp infection and without symptom of digestive tract were randomly divided into two groups： observation group （310 eases） and control group （308 cases）. The observation group used non-drug toothpaste combined with fluorine methods material for brushing test. Among which 82 cases with oral cavity ulcer additionally received modified chitin xanthan gum spray on oral ulcer. The latter （308 cases） used blank toothpaste as a control, cleaning the mouth with toothpaste after dinner ev- ery day. The Hp positive rate, average Hp eradication time and indicators such as oral cavity ulcer healing condition in two groups were compared. Results： In the treatment of after 30 d, 90 d, the oral Hp positive rate of observation group and control group was 53.55% ,98.70% and 7.94% ,94.59% , the difference was statistically significant （ P 〈0.01 ）. In the respect of treatment of oral ulcer, after 7 days and 30 days, the effective rate of observation group and control group was 76.74% ,15.19% and 100% ,32.91% , the difference was statistically significant （ P 〈0.01 ）. There had no severe adverse reactions occurred in the process of treatment in observation group. Conclusion： Fluorine methods material could effectively decrease the positive rate of oral Hp infection, the treatment eombinded with modified chitin xanthan gum could shorten the time of oral ulcer healing, it had important value to control the oral Hp infection.
