小承气汤治疗术后早期炎性肠梗阻的疗效观察

摘　　要：目的:探讨小承气汤治疗术后早期炎性肠梗阻的临床疗效。方法:将78例术后早期炎性肠梗阻患者随机分为两组,每组39例,对照组予以西医常规治疗,治疗组在对照组基础上予小承气汤治疗,连续治疗7 d后观察比较两组患者临床疗效、血浆炎性反应改善(CRP、TNF-α及IL-6)及胃肠道功能恢复(肠鸣音恢复正常时间、首次肛门排气时间、首次排便时间)等情况。结果:治疗组总有效率为89.7%,对照组总有效率为69.2%,治疗组优于对照组,差异有统计学意义(P<0.05);治疗组患者胃肠功能各指标恢复情况均优于对照组患者,差异有统计学意义(P<0.05);治疗后治疗组患者血浆TNF-α、CRP均低于对照组,差异有统计学意义(P<0.05)。结论:小承气汤治疗术后早期炎性肠梗阻有显著临床疗效,可有效控制机体术后炎性反应,促进患者胃肠功能恢复。Objective： To investigate the effects of Xiaochengqi decoction on early inflammatory intestinal obstruction after surgical treatment. Methods： The 78 patients with early inflammatory intestinal obstruction after surgical treatment were randomly divided into two groups, 39 cases in each. The control group was treated with routine treatment of Western medicine; the treatment group was given Xiaochengqi Decoction. After 7 days of continuous treatment, the clinical efficacy, plasma inflammatory indexes （CRP, TNF-a and IL-6）, and gastrointestinal function （bowel sounds recovery normal time, first anal exhaust time, the first defecation time） were observed. Results： The clinical total effective rate was 89.7% in treatment group and 69.2% in the control group, the effects of the treatment group were better than the control group （P〈0.05）. The improvement of those indexes of gastrointestinal function in treatment group were better than control group （P〈0.05）. The content of plasma TNF-α CRP was lower than the control group after treatment （P〈0.05）. Conclusions： Xiaochengqi decoction has good efficiacy on early inflammatory intestinal obstruction after surgical treatment, can effectively improve the body＇s postoperative inflammatory reaction and promote gastrointestinal recovery.
