充血性心力衰竭气虚血瘀证临床特点研究

摘　　要：目的探讨充血性心力衰竭（CHF）气虚血瘀证的临床特点。方法收集上海中医药大学附属龙华医院、岳阳中西医结合医院、普陀中心医院2010年12月—2012年9月收治的气虚血瘀证CHF病人1 451例,探讨CHF气虚血瘀证发病年龄、性别差异、原发疾病情况以及与实验室指标的相关性等。结果 CHF气虚血瘀证多见于80岁以上病人;50岁~70岁男性以及70岁以上女性病人较为多发;多见冠心病、房颤、2型糖尿病、脑梗死等并发疾病;中医病名多为胸痹心痛病;临床表现多为胸闷、气促、心悸、下肢浮肿、乏力、胸痛、小便量少、舌红、苔白、脉细等;心功能Ⅲ级以及BNP在1 006pg/mL以上的病人较多见。结论 CHF气虚血瘀证在发病年龄、性别、原发疾病情况以及与实验室指标的相关性等方面有一定的特点。Objective To investigate the clinical characteristics of congestive heart failure（CHF）with qi-deficiency and blood-stasis syndrome by clinic retrospective study.Methods Clinic retrospective cases were collected from Longhua Hospital,Yueyang Hospital of Integrated Traditional Chinese and Western Medicine,and Putuo Central Hospital affiliated to Shanghai University of Traditional Chinese Medicine from December,2010 to September,2012.The correlations of qi-deficiency and blood-stasis syndrome of CHF and age,gender,primary illnesses,and laboratory indexes had been studied and summarized.Results CHF qi-deficiency and blood-stasis syndrome was mostly seen in patients aged over 80,especially in male aged 50 to 70 and female aged over 70.The common primary diseases were coronary heart disease,atrial fibrillation,diabetes and cerebral infarction.In traditional Chinese Medicine,CHF belonged to the category of chest obstruction and heartache disease.Its clinical manifestations were chest distress,shortness of breath,palpitation,edema of lower limbs,lacking in strength,chest pain,scanty urine,red tongue,white fur,and thin pulse.Patients with this syndrome pattern of CHF were mostly in cardiac function Ⅲ level,and their inspection results of brain natriuretic peptide（BNP）were mostly over 1 006 pg/mL.Conclusion There are definite characteristics of CHF with qi-deficiency and blood-stasis syndrome in relations with age,gender,primary illnesses,and laboratory indexes.
