下腔静脉缺如胎儿超声心动图诊断应用

摘　　要：目的:探讨下腔静脉缺如胎儿超声心动图诊断的价值及其特点。材料与方法:回顾性分析13例2004年3月-2014年11月在本院行超声检查诊断为下腔静脉缺如的胎儿的超声表现,并与下腔静脉正常的胎儿超声表现比较。结果:共诊断13例下腔静脉缺如胎儿,有4例不伴其他心血管畸形,产前诊断未发现心脏结构异常,均足月分娩,新生儿超声联合CT检查除1例合并内脏反位,其余均未见生长发育异常;有9例伴有心脏畸形,均引产,尸体解剖3例,结果与超声诊断一致,并且均存在左侧异构。结论:产前超声检查胎儿四个典型切面(上腹部横切面、四腔心切面、胸腹部斜冠状切面黑右心房纵切面)对诊断胎儿下腔静脉缺如的诊断具有重要意义。超声显示下腔静脉缺如合并先天性心脏畸形,则有潜在左侧异构畸形的风险。Purpose： To explore the diagnosis value and characteristics of the absence of inferior vena cava by fetal ultrasound chart. Materials and Methods： 13 cases of Patients in the hospital between March 2004 and November 2014 were diagnosed by ultrasound examination for the absence of inferior vena cava , analyzed retrospectively fetal ultrasound echocardiography of inferior vena cava patients, and compared with normal patients. Results ： 13 cases of fetal were diagnosed with inferior vena cava absence. Among them, 4 cases without other cardiovascular malformation were term delivery , their cardiac structure were not found abnormal by prenatal diagnosis, and newborn ultrasound combined CT examination found 1 case combined visceral reaction, the rest were not seen abnormal growth and development; 9 cases were associated with heart or other visceral malformations, all induced labor, autopsy results were consistent with the ultrasonic diagnosis, and all of them were left isomerism. Conclusion ： Prenatal ultrasound examination of fetal four typical sections （ upper abdominal transverse view, four chamber view of the heart, chest and abdomen oblique coronal section black right atrial longitudinal section） on the diagnosis of fetal absence of inferior vena cava diagnosis has important significance. Ultrasound showed the absence of inferior vena cava with congenital heart deformity, the risk of potentially heterogeneous deformity, especially the left isomerism deformity.
