摘 要:目的:探讨双源CT后处理VR成像在骨关节骨折中的临床应用价值。材料与方法:选取我院在2014年1月至2015年1月期间收治的78例四肢骨关节骨折患者,采用双源CT双能量扫描,2个球管电压的扫描参数分别为100k V和140k V,并运用容积显示(VR)法进行图像重建,并根据轴位扫描图像进一步分析VR重建图像对骨折碎片,骨折线,四肢脱位情况以及关节面损伤程度的显示情况。结果:双源CT后处理VR成像能够清晰的显示患者的骨折线,且能够多方位、立体化显示患者的骨折及关节脱节部位情况,以及能够对患者骨折劈裂方向以及塌陷面积进行准确评估,且该方法的图像质量为优者高达89.74%(70/78)显著高于模拟常规法成像60.26%(47/78),两种方法比较差异存在统计学意义(Z=-5.64,P<0.01)。结论:双源CT扫描后VR成像,能够对骨关节的骨折情况获取高质量的图像,安全性较高,有助于临床医生对患者的骨折情况进行准确判断,并制定合理、有效的手术方案,也有助于对患者的预后康复进行良好评估,值得临床推广应用。Purpose: To investigate the clinical value of Dual-source CT post-processing VR imaging in the joint fracture. Materials and Methods: 78 cases of patients with fracture of limb joints who adopted dual source CT dual energy scanning in our hospital from 2014 January to 2015 January were selected. The scanning parameters of the 2 ball tube voltage were 100 kv and 140 kv respectively, and using volume rendering method (VR) for image reconstruction, and according to the axis of scan images further analysis VR image reconstruction of fracture fragments, fracture iine, the dislocation of limbs as well as the display of the joint surface damage degree. Results: Dual-source CT post-processing VR imaging could clearly display the patient' s bone fracture line, and were able to all-round, three-dimensional display fractures as well as joints between parts of the patients. Dual-source CT post-processing VR imaging had the ability to accurately evaluated the direction of the patients with fracture splitting and collapsing area. Besides, the method of image quality for instituting was as high as 89. 74% (70/ 78 ), which was significantly higher than normal simulation method imaging of 60. 26% (47/78), and two methods had signif- icant differences (Z = 5.64, P 〈 0. 01 ). Conclusion: Dual-source CT post-processing VR imaging is able to obtain high quality images of joint fracture of obtaining, which is higher security and can help clinical doctors to accurately judge the fracture profile of patients. By this method, it is good for clinical doctors to formulate rational and effective operation plan, and can also help to evaluate the prognosis of patients with rehabilitation. Thus it is worthy of clinical popularization and applica- tion.
