120例足月妊娠不同方法促宫颈成熟疗效分析

摘　　要：目的：比较地诺前列酮与缩宫素在足月妊娠促宫颈成熟治疗中的疗效。方法选择妊娠足月要求阴道试产有引产指征而无禁忌证的孕妇120例，分为地诺前列酮组（68例）和缩宫素组（52例），分别给予阴道使用地诺前列酮栓和静脉滴注小剂量缩宫素促宫颈成熟治疗，分析其疗效及妊娠结局。结果地诺前列酮组临产率（85．29％）高于缩宫素组（57．69％），临产时间〔（10．35±5．51）h〕短于缩宫素组〔（23．65±28．29）h〕，阴道分娩率（75．00％）高于缩宫素组（55．77％），差异有统计学意义（P＜0．05）。2组总产程时间、胎儿窘迫发生率、急产发生率比较差异无统计学意义（P＞0．05）。结论地诺前列酮栓在足月妊娠促宫颈成熟治疗时疗效肯定，安全性好，优于缩宫素。Objective To compare the efficacies of dinoprostone and oxytocin for cervical ripening in full-term pregnant women.Methods A total of 120 full-term pregnant women who required induction of labor without any con-traindications were enrolled into the current study.They were divided into a dinoprostone group （ n=68） who were vagi-nally given dinoprostone suppositories and a oxytocin group （n=52） who were ivgtt administrated with a small dose of ox-ytocin.Then, both groups were compared for their efficacies and clinical outcomes.Results The dinoprostone group produced a successful labor rate （85.29%）, which was obviously higher than that of the oxytocin group （57.69%） （P〈0.05）.The average labor time was [（10.35 ±5.51）h] in the dinoprostone group, which was remarkably shorter than that in the oxytocin group [（23.65 ±28.29）h] （P〈0.05）.The vaginal delivery rate was 75.00%（51/68） in the di-noprostone group, which was markedly higher than that in the oxytocin group 55.77%（29/52） （P〈0.05）.There was no significant difference in the total labor time, the rate of fetal distress and the rate of partus precipitatus between the two groups （P〉0.05）.Conclusion Dinoprostone vaginal suppositories produce superior efficacy over oxytocin when ap-plied for cervical ripening in full-term pregnant women, with good safety.
