α-硫辛酸联合前列地尔、甲钴胺治疗老年糖尿病周围神经病变的临床疗效研究

摘　　要：目的探究α-硫辛酸联合前列地尔、甲钴胺治疗老年糖尿病周围神经病变(DPN)的临床疗效。方法选取冀中能源邢台矿业集团有限责任公司总医院2012年2月—2014年5月收治的老年DPN患者87例,随机分为观察组45例和对照组42例。两组患者均给予常规降糖治疗,对照组患者在此基础上加用甲钴胺和前列地尔治疗;观察组患者在对照组基础上联合α-硫辛酸治疗。连续治疗2周为1个疗程,两组患者均治疗1个疗程后评价临床疗效。比较两组患者的临床疗效,治疗前后正中神经、腓总神经运动神经传导速度(MCV)及正中神经、腓总神经、腓肠神经感觉神经传导速度(SCV),并观察治疗期间不良反应发生情况。结果观察组患者临床疗效优于对照组(u=11.784,P<0.05)。两组患者治疗前正中神经MCV和SCV、腓总神经MCV和SCV、腓肠神经SCV比较,差异无统计学意义(P>0.05);观察组患者治疗后正中神经MCV和SCV、腓总神经MCV和SCV、腓肠神经SCV均高于对照组(P<0.05)。两组患者治疗期间不良反应发生率比较,差异无统计学意义(P>0.05)。结论α-硫辛酸联合前列地尔、甲钴胺治疗老年DPN患者的临床疗效确切,且不良反应轻微。Objective To investigate the clinical effect of α - lipoic acid combined with alprostadil and mecobalamin on aged diabetic peripheral neuropathy. Methods From February 2012 to May 2014, a total of 87 aged patients with diabetic peripheral neuropathy were selected in the General Hospital of Xingtai Mining Group Limited Liability Company, Jizhong Energy Resources, and they were randomly divided into control group （ n = 42 ） and observation group （ n = 45 ）. Patients of both groups received routine hypoglycemic treatment, and patients of control group received extra alprostadil and mecobalamin, while patients of observation group received extra α - lipoic acid combined with alprostadil and mecobalamin. Both groups treated for 2 weeks as course. Clinical effect, motor nerve conduction velocity （MCV） of median nerve and common fibular nerve before and after treatment, sensory nerve conduction velocity （ SCV ） of median nerve, common fibular nerve and sural nerve before and after treatment were compared between the two groups, and the incidence of adverse reactions during treatment was observed. Results The clinical effect of observation group was statistically significantly better than that of control group （ u = 11. 784, P 〈 0. 05 ）. No statistically significant differences of MCV of median nerve or common fibular nerve, SCV of median nerve, common fibular nerve or sural nerve was found between the two groups before treatment （P 〉 0.05） ; MCV of median nerve and common fibular nerve, SCV of median nerve, common fibular nerve and sural nerve of observation group were statistically significantly higher than those of control group after treatment （ P 〈 0. 05 ）. No statistically significant differences of incidence of adverse reactions during treatment was found between the two groups （ P 〉 0. 05 ）. Conclusion α - lipoic acid combined with alprostadil and mecohalamin has certain clinical effect on aged diabetic peripheral neuropathy, and the adverse reactions is mild.
