外周血中性粒细胞与淋巴细胞比值与2型糖尿病肾病的相关性研究

摘　　要：目的:探讨外周血中性粒细胞与淋巴细胞比值(NLR)与2型糖尿病肾病(DN)的相关性。方法:将2014年1-7月本院收治的2型糖尿病患者330例根据24 h尿微量白蛋白(UMA)水平分为无糖尿病肾病组(N组)、早期糖尿病肾病组(EDN组)、临床糖尿病肾病组(CDN组),收集患者一般临床指标及生化指标,分析NLR在各组之间的差异及与DN的关系。结果:CDN组、EDN组白细胞计数、中性粒细胞计数及NLR均高于N组,CDN组NLR高于EDN组,比较差异均有统计学意义(P<0.01);CDN组淋巴细胞计数低于N、EDN组,比较差异均有统计学意义(P<0.05)。NLR与UMA呈正相关(r=0.215,P<0.01)。NLR是2型糖尿病肾病的危险因素[OR=3.568,95%CI(1.403,9.073),P<0.01]。结论:外周血NLR与2型糖尿病肾病具有显著的相关性,是2型糖尿病肾病发生的独立危险因素。Objective：To explore the relationship between peripheral blood neutrophil-to-lymphocyte ratio（NLR） and the type 2 diabetic nephropathy.Method：330 patients with type 2 diabetic in our hospital from January 2014 to July 2014 were divided into non-diabetic nephropathy group（N group）,early diabetic nephropathy group（EDN group） and clinical diabetic nephropathy group（CDN group）according to their 24 hours urine microalbumin（UMA）levels.The clinical and biochemical data were collected for comparing the differences in NLR among the three groups and analyzing the relationship between NLR and DN.Result：The white blood cell count,neutrophil count and NLR in the CDN,EDN group were significantly higher than those in the N group,the differences were statistically significant（P〈0.01）.The NLR in the CDN group was significantly higher than that in the EDN group,the difference was statistically significant （P〈0.01）.However,the lymphocyte count in the CDN group was lower than that in the N group and EDN group,the＆amp;nbsp;differences were statistically significant（P〈0.05）.NLR was positively related with UMA（r=0.215,P〈0.01）.NLR was a risk factor of type 2 diabetic nephropathy[OR=3.568,95%CI（1.403,9.073）,P〈0.01].Conclusion：There are significantly correlations between peripheral blood NLR and type 2 diabetic nephropathy.NLR is the independent risk factor for type 2 diabetic nephropathy.
