超声应变率成像评价原发性高血压血压变异性及昼夜节律对左室舒张功能的影响

摘　　要：目的:探讨超声应变率成像评价原发性高血压血压变异性及昼夜节律对左室舒张功能的影响。方法:选取原发性高血压患者60例作为研究组,同期选取健康人群60例作为对照组,全部患者均采用动态24 h血压监测、常规超声心动图与超声应变率成像检查,比较两组血压变异性及昼夜节律、左室舒张功能的差异。结果:研究组d-SBP、d-DBP、n-SBP、n-DBP水平均明显高于对照组(P<0.05)。研究组室间隔舒张末厚度(IVSTd)、左心室后壁舒张末厚度(LVPWd)、左心室质量指数(LVMI)均明显高于对照组,研究组舒张早期/晚期最大血流速度比值(E/A)明显低于对照组(P<0.05),两组左心室射血分数(LVEF)比较差异无统计学意义(P>0.05)。研究组平均舒张早期峰值速度(mVe)、平均舒张早期/晚期峰值速度比值(mVe/mVa)、平均舒张早期峰值应变率(mSre)、平均舒张早期/晚期峰值应变率比值(mSre/mSra)均明显低于对照组,平均舒张晚期峰值速度(mVa)、平均舒张晚期峰值应变率(mSra)明显高于对照组(P<0.05)。结论:原发性高血压血压变异性及昼夜节律改变严重影响左室舒张功能,其中超声应变率成像对左室舒张功能评价具有重要的价值。Objective： To investigate the ultrasonography strain rate imaging in evaluation of blood pressure variability and circadian rhythm on left ventricular diastolic function for essential hypertension. Method：60 essential hypertension were selected as study group, and 60 healthy people were selected as control group, and all were given 24 hours blood pressure monitoring, echocardiography and ultrasonography srain rate imaging,blood pressure variability,circadian rhythm and left ventricular diastolic function were compared between the two groups. Result：The d-SBP, n-SBP, d-DBP and n-DBP levels of the study group were significantly higher than the control group（P〈0.05）. IVSTd, LVPWd and LVMI of the study group were significantly higher than the control group（P〉0.05）, and LVEF between the two groups had no statistically significant difference（P〈0.05）.The m Ve, m Ve/m Va,m Sre, m Sre/m Sra of the study group were significantly lower than the control group, m Va,m Sra of the study group were significantly higher than the control group（P〈0.05）. Conclusion： The changes of blood pressure variability and circadian rhythm have a serious influence on left ventricular diastolic function for essential hypertension, and ultrasonography strain rate imaging has important value in evaluating of left ventricular diastolic function.
