自制腹腔镜腹膜推进器在腹腔镜下腹膜阴道成形术中的应用

摘　　要：目的:探索自制腹腔镜腹膜推进器对提高腹腔镜下腹膜代阴道成形术的成功率和成形效果的价值。方法:选择本院2003年1月-2015年2月收治的44例先天性无阴道患者作为研究对象,将2003年1月-2007年8月未使用自制腹腔镜腹膜推进器,行腹腔镜下腹膜代阴道成形术的25例作为对照组,2007年8月-2015年2月应用自制腹腔镜腹膜推进器行腹腔镜下腹膜代阴道成形术的19例作为研究组,观察比较两组术中及术后情况。结果:研究组手术时间、术中出血量、胃肠功能恢复时间、下床活动时间及术后并发症均少于对照组,比较差异均有统计学意义(P<0.05);研究组阴道长度长于对照组,排异反应少于对照组,比较差异均有统计学意义(P<0.05)。结论:自制腹腔镜腹膜推进器用于腹腔镜下腹膜阴道成形术,简化了手术操作,缩短了手术时间,且不易损伤脏器,极大提高了手术成功率和成形效果。Objective： To investigate the value of homemade ]aparoscopic peritoneal propeller in improving the success rate and the forming effect of peritoneal vaginoplasty.Method： 44 patients with congenital absence of vagina in our hospital from January 2003 to February 2015 were selected as the research objects.25 patients who were given laparoscopic peritoneal vaginoplasty without homemade laparoscopic peritoneal propeller from January 2003 to August 2007 were as the control group.19 patients who were given laparoscopic peritoneal vaginoplasty using homemade laparoscopic peritoneal propeller from August 2007 to February 2015 were as the research group. The intraoperative and postoperative situation of the two groups were observed and compared.Result： The operation time, the amount of bleeding in the operation, the time needed for the recovery of gastrointestinal function, the time of bff-bed and the incidence of postoperative complications in the research group were less than those in the control group, the differences were statistically significant （P〈0.05） .The length of the vagina in the research group was longer than that in the control group, the rejection reaction of the research group was less than that of the control group, the differences were statistically significant （ P〈0.05 ） .Conclusion： Homemade laparoscopic peritoneal propeller used in the laparoscopic peritoneal vaginoplasty can simplify the operation, shorten the operation time, avoid damage to the organs and greatly improve the success rate and the forming effect.
