剖宫产术后再次妊娠阴道分娩与剖宫产对母婴的影响比较

摘　　要：目的观察及比较剖宫产术后再次妊娠阴道分娩与剖宫产对母婴的影响。方法选取2013年6月~2015年4月于本院进行分娩的86例剖宫产术后再次妊娠孕妇为研究对象,将其根据分娩方式的不同分为A组(剖宫产组)43例和B组(阴道分娩组)43例,然后将两组产妇出血量、产褥感染率、住院时间、分娩前后的疼痛评分及新生儿5min Apgar评分、肺炎发生率进行比较。结果 B组的出血量小于A组(t=6.874),产褥感染率低于A组(x^2=6.641),住院时间短于A组(t=5.868),产后1d及3d疼痛评分均好于A组(x^2=6.386,5.973),差异有统计学意义(P均<0.05),而两组新生儿5min Apgar评分及肺炎发生率则无统计学差异(x^2=0.021,0.1101,P>0.05)。结论剖宫产术后再次妊娠阴道分娩对产妇的不良影响相对更小,而阴道分娩与剖宫产对新生儿的影响无明显差异。Objective To observe and compare the influence of vaginal delivery and ceasarean section for the mother and infant with repeated pregnancy after cesarean section. Methods 86 pregnant women with repeated pregnancy after cesarean section in our hospital from June 2013 to April 2015 were selected as the research object, and they were divided into group A（cesarean section group）43 cases and group B（vaginal delivery group）43 cases according to the differences of delivery methods, then the blood loss, puerperal infection rate, hospitalization time and pain score before and after the delivery of delivery women,5min Apgar and pneumonia rate of neonatus of two groups were compared. Results The blood boss of group B was less than that of group A（t=6.874）,puerperal infection rate was lower than that of group A（x^2=6.641）, hospitalization time was shorter than that of group A（t=5.868）, the pain score at 1st and 3rd day after the delivery were better than those of group A（x^2=6.386, 5.973, all P〈 0.05）, while the 5min Apgar and pneumonia rate of two groups had no significant differences（x^2=0.021, 0.1101, P〉 0.05）. Conclusion The bad influence of vaginal delivery for the mother with repeated pregnancy after cesarean section are relatively smaller, and the influence of vaginal delivery and ceasarean section for the neonatus have no obvious differences.
