摘  要:目的:探讨右美托咪定联合瑞芬太尼TCI(Target controlled infusion)对ICU患者镇静期应激反应的影响。方法:选取ICU 60例接受机械通气需要镇静的患者,随机分为右美托咪定组(M)、瑞芬太尼组(R)、联合用药组(MR);检测各组不同时间点患者的血清皮质醇、IL-6浓度。结果:MR组在T4~T9时间点均较单独用药组皮质醇浓度明显降低,差异有统计学意义(P〈0.05);MR组在T3~T9时间点均较单独用药组IL-6血清浓度明显降低,差异有统计学意义(P〈0.05)。结论:右美托咪定联合瑞芬太尼TCI联合用药能有效遏制ICU患者应激反应,降低并发症。Objective: To investigate the influence of the stress response for dexmedetomidine and remifentanil TCI (target controlled infusion) on ICU patients in sedative period. Methods: A total of 60 patients receiving mechanical ventilation and requiring sedation were randomly divided into three groups, group given dexmedetomidine (M), remifentanil group (R), combination group (MR). Concentration of serum cortisol and IL-6 were tested in each group patients at different time points. Results: Compared with medication alone group, cortisol concentrations of patients in MR at T4-T9 time point were significantly lower (P〈0.05). Serum IL-6 concentrations of patients in R at T3-T9 time point were significantly lower (P〈0.05). Conclusions: Dexmedetomidine combined with remifentanil TCI can control high stress reaction of ICU patients, and reduce the complications.
