摘  要:目的研究磁共振扩散加权成像(DWI)评价兔VX2肝移植瘤模型经皮酒精注射术(PEI)疗效及PEI对残余肿瘤微血管生成的影响。方法将26只接种成功的兔VX2肝移植瘤模型随机分为实验组和对照组,每组各13只。实验组CT引导下行PEI,对照组不予处理,术后1周行DWI检查,比较实验组肿瘤活性区及坏死区表观弥散系数(ADC)的差异、2组活性肿瘤区ADCperf和微血管密度(MVD)计数的差异,并分析ADCperf与MVD计数的相关性。正态性分析采用Shapiro-Wilk检验,计量资料组间比较采用Wilcoxon秩和检验,ADCperf与MVD计数的相关性比较采用Pearson相关分析。结果实验组坏死区与活性肿瘤区ADCtotal值分别为[1.674(1.340~2.571)]×10-3mm2/s、[1.296(1.120~1.972)]×10-3mm2/s,差异有统计学意义(Z=2.052,P=0.040);实验组与对照组活性肿瘤区ADCperf值分别为[0.568(0.381~1.873)]×10-3mm2/s、[0.947(0.518~1.476)]×10-3mm2/s,MVD计数分别为(23.641±6.138)、(23.487±3.466),2组比较差异均无统计学意义(Z值分别为0.487、-0.128,P值分别为0.626、0.898);2组肿瘤活性区ADCperf与MVD计数均呈正相关(r值分别为0.583、0.593,P值分别为0.037、0.033)。结论 DWI可准确评价兔VX2肝移植瘤PEI的疗效并在一定程度上反映其微血管生成情况,PEI术后1周对残余肿瘤微血管生成无明显影响。Objective To investigate the application of diffusion - weighted magnetic resonance imaging ( DW - MRI) for the evaluation of the therapeutic effect of percutaneous ethanol injection (PEI) and its effect on micro - angiogcnesis in residual tumor in a rabbit VX2 liver tnmor model. Methods A total of 26 rabbits with VX2 liver tumor were randomly divided into experimental group and control group, with 13 rabbits in each group. CT -guided PEI was performed for the experimental group, and no treatment was performed for the control group. DW - MRI was performed at 1 week after operation. Apparent diffusion coefficient (ADC) ADCtota! in the active tumor areas and necrosis ar- eas were compared within the experimental group, ADCperf and microvascular density (MVD) count in the active tumor areas were compared between the two groups, and the correlation between ADCperf and MVD count was analyzed. The Shapiro -Wilk test was applied for normali- ty analysis, the Wilcoxon rank sum test was applied for comparison of continuous data between the two groups, and the correlation between ADCperf and MVD count was analyzed by the Pearson correlation coefficient. Results In the experimental group, ADCtota1 showed a signifi- cant difference between the necrosis areas and the active tumor areas ( [ 1. 674 ( 1. 340 - 2. 571 ) ] × 10 -3 mm2/s vs [ 1. 296 ( 1. 120 - 1. 972) ]×10 -3 mm2/s; Z =2. 052, P =0.040). ADCperf and MDV count in the active tumor areas showed no significant differences be- tween the experimental group and the control group [0.568(0.38l -1. 873)] x 10-3 nllnU/s VS [0. 947(0.518 -1. 476)] × 10-3 mm2/s, Z = O. 487, P = O. 626; 23. 641 ± 6. 138 vs 23.487 _+ 3.466; Z = - 0. 128, P = 0. 898). In both groups, ADCperf in the active tumor areas was positively correlated with MDV count ( r = O. 583 and 0.593, P = 0. 037 and 0. 033). Conclusion DW - MRI can accurately assess the therapeutic effect of PEI and reflect the condition of micro - angiogenesis in a rabbit VX2 liver tum
