摘  要:目的 探讨血清α1微球蛋白、胎儿血红蛋白对子痫前期发病的预测价值。方法 选取2013年10月-2015年2月在医院立卡产检的无妊娠合并症、并发症的单胎初产妇346例作为研究对象,其中正常对照组239例,妊娠期高血压组40例,轻度子痫前期组38例及重度子痫前期组29例。对所有产妇进行血清α1微球蛋白、胎儿血红蛋白检测,观察4组产妇的检测结果。结果 正常对照组及妊娠期高血压组产妇的血清α1微球蛋白水平及胎儿血红蛋白水平(Hb F%)相比差异无统计学意义(P〉0.05);轻度子痫前期组及重度子痫前期组产妇的血清α1微球蛋白水平及胎儿Hb F%较妊娠期高血压组显著提高,差异有统计学意义(P〈0.05);与轻度子痫前期组相比,重度子痫前期组产妇的血清α1微球蛋白水平及胎儿Hb F%显著提高,差异有统计学意义(P〈0.05)。4组产妇妊娠22~26周时的子宫动脉(左、右)RI值、胎儿的大脑中动脉RI、胎儿脐血流S/D值、早产率及阿氏评分差异均有统计学意义(P〈0.05)。结论 血清α1微球蛋白、胎儿血红蛋白对子痫前期发病具有重要预测价值,值得深入研究。Objective To explore the value of serum alpha 1 microglobulin and fetal hemoglobin in prediction of preeclampsia. Meth- ods A total of 346 primiparous women (single birth, without pregnancy complications) establishing cards in the hospital from October 2013 to February 2015 were selected and divided into normal control group (239 cases), gestational hypertension group (40 cases), mild pre- eclampsia group (38 cases), and severe preeclampsia group (29 cases) . Serum alpha 1 microglobulin and fetal hemoglobin levels in the four groups were detected and observed. Results There was no statistically significant difference in the levels of serum alpha 1 microglobulin and fetal hemoglobin between normal control group and gestational hypertension group (P〉0. 05 ) ; the levels of Serum alpha 1 microglobnlin and fetal hemoglobin in mild preeclampsia group and severe preeclampsia group were statistically significantly higher than those in gestational hypertension group (P〈0. 05) ; the levels of serum alpha 1 microglobulin and fetal hemoglobin in severe preeclampsia group were statistically significantly higher than those in mild preeclampsia group ( P〈0. 05 ) . There were statistically significant differences in RI values of left u- terine artery and right uterine artery during 22-26 gestational weeks, RI value of fetal middle cerebral artery, fetal umbilical S/D ratio, inci- dence rate of premature birth, and Apgar score among the four groups. Conclusion Serum alpha 1 microglobulin and fetal hemoglobin have significant predictive value for preeclampsia, which are worthy of further study.
