摘 要:目的探讨母亲妊娠期高血压疾病对新生儿脑发育状况的影响。方法应用颅脑超声检测90例高血压疾病孕妇分娩新生儿(病例组)的额叶厚度、大脑横径、额上回宽度、脑岛面积和周长、脑灰质及脑白质发育,并与198例无高危围产因素孕妇分娩新生儿(对照组)进行比较。结果对照组中足月儿额叶厚度和大脑横径、脑岛面积和周长、额叶白质平均灰度值均大于早产儿,额上回宽度小于早产儿(均P<0.05)。病例组足月儿与早产儿额叶厚度和大脑横径均小于对照组足月儿和早产儿,额上回宽度分别大于对照组,脑岛面积和周长分别小于对照组,差异均有统计学意义(均P<0.05),且部分新生儿存在不同程度的脑室增宽。结论母亲妊娠期高血压疾病可能造成新生儿脑发育成熟度欠佳,颅脑超声可进行测量并予以评价,能为临床早期干预及治疗提供依据。Objective To study the effect of hypertensive disorder complicating pregnancy on neonate brain development. Methods Thickness of the frontal lobe, the brain diameter, the width of the superior frontal gyrus, insular area and circle, cerebral grey matters ,white matters were measured in 90 infants born by women with hypertensive disorder complicating pregnancy by brain uhrasonography and image analysis system, and the results were compared with those of 198 infants born by women without high risk factors during perinatal period (control group). Results In the control group, the thickness of the frontal lobe of full-term infants was thicker and the brain diameter was longer than those of preterm neonates, the width of the superior frontal gyrns was narrower, the insular area was larger and the insular circle was longer, the average values of grey density in the white matters of frontal lobe was higher(all P〈0.05 ). In the group of the hypertensive disorder complicating pregnancy, the thickness of the frontal lobe of the full-term and preterm infants was thinner and the brain diameter was shorter than those in control group, the width of the superior frontal gyrns was wider, the insular area was smaller and the insular circle was shorter (all P〈0.05 ).And partial infants appeared to have enlarged cerebral ventricles in different degree. Conclusion Hypertensive disorder complicating pregnancy may result in poor brain development of their infants, it could be evaluated and measured by brain uhrasonography, which can provide basis for clinical intervention and treatment at early stage.
